瘢痕疙瘩放射性核素敷贴治疗中国专家共识 被引量:1

Chinese expert consensus on the treatment of keloids via radionuclide application therapy

摘要 瘢痕是皮肤的常见病和多发病,通常是指皮肤创伤后愈合过程中形成的纤维组织(瘢痕组织)。瘢痕疙瘩是指异常增生的瘢痕组织突出皮肤表面,导致皮肤的异常质地和不规则外观,类似疙瘩。大部分瘢痕疙瘩是由手术或局部毛囊炎或痤疮所致。瘢痕疙瘩有时疼痛难忍,严重影响患者的身心健康及生活质量。瘢痕疙瘩应用常规方法治疗容易反复增生,是临床治疗难度极大的一种疾病。放射性核素敷贴治疗是预防瘢痕术后复发的一线治疗方法。放射性核素敷贴治疗在我国应用了半个多世纪,实践证明其安全性高,^(90)Sr/^(32)P释放β射线,在组织中具有射程短、电离能力强等特点,不会造成真皮层损伤,更不会对全身有影响。^(90)Sr/^(32)P敷贴治疗具有简便、安全、无痛、疗效确切等优点,近年来其在临床中的应用逐渐受到重视。本共识旨在为瘢痕疙瘩的放射性核素敷贴治疗提供指导性方案,以期优化该病的治疗方案、提高治疗效果、减少不良反应。 Scars are a prevalent dermatological condition,typically characterized by the formation of fibrous tissue(scar tissue)during the healing process following skin trauma.Keloids are defined as abnormally proliferative scar tissue that extends above the skin's surface,resulting in altered skin texture and an uneven appearance,often resembling nodules.Most keloids develop from surgical interventions,localized folliculitis,or acne.They can occasionally be extremely painful,significantly impacting physical and mental health and overall quality of life.Keloids treated with conventional methods frequently exhibit recurrent hypertrophic growth,presenting considerable challenges for clinical management.Radionuclide application therapy is a first-line treatment for preventing the recurrence of scars after surgery.This therapy has been utilized in China for over half a century,with clinical practice demonstrating a high safety profile.Isotopes such as^(90)Sr or^(32)P releaseβ-rays,which have a short penetration depth and strong ionizing ability in tissues,thereby preventing damage to the dermis and posing no systemic risks.Treatment with^(90)Sr or^(32)P application is straightforward,safe,painless,and highly effective,its application in clinical practice has gradually received attention recently.This consensus aims to provide guidelines for the treatment of keloids via radionuclide application therapy,with the goals of optimizing treatment protocols,enhancing therapeutic outcomes,and minimizing adverse effects.
