摘要
目的观察针对气滞血瘀型肩袖损伤患者,在实施关节镜下肩袖修补手术后,桃红四物汤加味联合浮针对其肩关节疼痛状况以及功能恢复情况的影响。方法选取2023年9月—2024年12月期间就诊于安徽中医药大学第一附属医院骨伤二科收治入院并行肩袖修补术的60名辨证属气滞血瘀型肩袖损伤的患者,采用随机对照设计的原则将其分成治疗组与对照组,每组患者各30例。对照组术后给予常规抗炎镇痛处理,治疗组术后增加桃红四物汤加味联合浮针治疗。评估两组治疗前后的临床疗效、评估患者治疗前后的肩关节疼痛视觉模拟评分法(Visual Analog Scale,VAS)评分,肩关节功能恢复效果(洛杉矶加州大学关节功能评分),肩关节活动度,IL-6、TNF-α。结果治疗组相对于对照组治疗的临床疗效高(P<0.05)。治疗组VAS评分低于对照组(P<0.05),治疗组肩关节活动度及肩关节功能恢复效果均优于对照组(P<0.05),治疗组IL-6、TNF-α表达较对照组明显降低(P<0.05)。结论桃红四物汤加味联合浮针治疗可以有效缓解进行肩袖修补术的气滞血瘀型肩袖损伤患者肩关节的早期疼痛情况,更好地恢复肩关节功能,改善关节活动,有效抑制炎症反应,改善局部循环,提高日常生活活动能力。
To evaluate Clinical Evaluation of Modified Taohong Siwu Decoction Combined with Floating Acupuncture in Managing Postoperative Rehabilitation for Rotator Cuff Injuries with Qi-Stagnation-Blood-Stasis Pattern.Methods A randomized controlled trial enrolled 60 patients diagnosed with rotator cuff injuries(Qi-stagnation-blood-stasis pattern)undergoing arthroscopic repair at the Second Orthopedics Department of Anhui University of Chinese Medicine First Affiliated Hospital(September 2023-December 2024).Participants were allocated equally into an observation group(n=30)receiving postoperative conventional anti-inflammatory analgesia plus modified Taohong Siwu Decoction and floating acupuncture therapy,and a control group(n=30)receiving standard care alone.Outcome measures included:Clinical efficacy rates.Visual Analog Scale(VAS)for shoulder pain.UCLA shoulder functional scoring system.Shoulder range of motion.IL-6 and TNF-αlevels.Results The observation group demonstrated:Significantly higher clinical efficacy versus controls(P<0.05).Lower VAS scores(P<0.05).Superior UCLA functional recovery(P<0.05)and greater shoulder range of motion improvements(P<0.05).Markedly reduced IL-6 and TNF-αexpression(P<0.05).Conclusion Modified Taohong Siwu Decoction Combined with Floating Acupuncture effectively alleviates early postoperative pain,enhances functional recovery,restores joint mobility,suppresses inflammatory responses,and improves circulatory dynamics in patients with Qi-stagnation-blood-stasis rotator cuff injuries,ultimately optimizing activities of daily living.
