摘要
目的评估汉化版数字健康素养工具(DHLI)在系统性红斑狼疮患者中的信效度,并确定其截断值。方法于2023年8—10月,采用DHLI汉化量表对380例SLE患者进行网络测评,对50例SLE患者进行线下测评。间隔2周后,对线下测评的50例SLE患者进行重测。采用探索性因子分析和验证性因子分析检验量表的结构效度;采用假设检验评估量表的聚合效度;以eHEALS汉化量表作为效标检验量表的同时效度;检验量表的内部一致性、分半信度和重测信度;采用受试者工作特征曲线(ROC)确定量表的灵敏度、特异度及截断值并计算ROC曲线下面积(AUC)。结果(1)DHLI包含4个公因子,累计方差解释率为78.26%;(2)四因子模型拟合良好,χ^(2)/df=2.191,RMSEA=0.075,IFI=0.962,CFI=0.961,TLI=0.952;(3)DHLI总分与SLE患者的受教育程度、家庭人均月收入、互联网使用年限均呈正相关。DHLI总分及操作技能、信息获取和利用、导航技能、内容生成技能维度与eHEALS总分呈正相关,相关系数为0.649、0.486、0.651、0.448和0.487(P<0.001);(4)量表总分及各维度的Cronbach'sα系数为0.776~0.954,Spearman-Brown分半信度为0.709~0.937,重测信度为0.913~0.988;(5)DHLI的截断值为51.5分,AUC为0.817。结论中文版DHLI具有良好的信效度,51.5分可作为DHLI测评SLE患者高、低数字健康素养水平的参考界值。
Objective To evaluate the reliability and validity of Chinese version of Digital Health Literacy Instrument(DHLI)in patients with systemic lupus erythematosus,determining the cut-off value.Methods A network evaluation was conducted on 380 SLE patients using the DHLI and an offline evaluation was conducted on 50 SLE patients from August to October 2023.After a 2-week interval,50 SLE patients evaluated offline were retested.Exploratory factor analysis and confirmatory factor analysis were used to test the construct validity of the scale.Hypothesis testing was selected to evaluate the convergent validity of the scale.eHEALS Sinicized Scale was used as a benchmark to test the simultaneous validity of the scale,verifying the internal consistency,split half reliability,and test-retest reliability of the scale.Receiver operating characteristic(ROC)curves was used to determine the sensitivity,specificity,and cutoff values of the scale and calculate the area under the ROC curve(AUC).Results(1)The DHLI contained four factors,accounting for 78.26%of the total variance.(2)The four-factor model fitted well,2/df=2.191,RMSEA=0.075,IFI=0.962,CFI=0.961,TLI=0.952.(3)The total score of DHLI was positively correlated with the education level of SLE patients,the per capita monthly income of families,and the years of Internet use.The total score of DHLI,operational skills,information acquisition and utilization,navigation skills,and content generation skills were positively correlated with the total score of eHEALS.The correlation coefficients were 0.649,0.486,0.651,0.448,and 0.487(P<0.001).(4)The DHLI had good internal consistency,with the total scale's and each dimension's Cronbach'sαranging from 0.776 to 0.954,the split-half reliability ranging from 0.709 to 0.937,the test-retest reliability ranging from 0.913 to 0.988.(5)The cut-off value of the DHLI was 51.5,with the area under curve of 0.817(95%:0.772~0.863).Conclusions The Chinese version of DHLI has good reliability and validity.A cut-off score of 51.5 was recommended for determining high and low levels of digital health literacy in SLE patients using DHLI.
