摘要
目的:探讨新生儿重症监护室(NICU)早产儿家属的疾病不确定感与心理韧性、社会支持的相关性。方法:选取2020年7月-2023年7月期间本院NICU内103例早产儿的家属作为研究对象。调查早产儿家属一般资料、家属疾病不确定感、心理韧性和社会支持情况,用Pearson相关系数分析NICU早产儿家属疾病不确定感与心理韧性、社会支持间的相关性,用二元logistic回归分析NICU早产儿家属疾病不确定感影响因素。结果:NICU早产儿家属疾病不确定感程度低水平16.5%、中水平39.8%、高水平43.7%,疾病不确定感总分83.91±12.12分,心理韧性总分67.24±9.69分、社会支持总分26.37±4.43分;疾病不确定感低水平者心理韧性、社会支持量表各维度得分及总分均高于中高水平者,疾病不确定感与心理韧性、社会支持水平均呈负相关(均P<0.05);回归分析,文化程度低、家庭收入低、早产儿病情严重、心理韧性低、社会支持低均为NICU早产儿家属疾病不确定感的影响因素(均P<0.05)。结论:本次调查的NICU早产儿家属疾病不确定感水平较高,与心理韧性、社会支持有相关性,提示社会应关注高危患儿家属心理健康问题,提高家属心理韧性及社会支持水平有助于减轻家属的疾病不确定感。
Objective:To explore the correlation between the disease uncertainty of family members of premature infants in neonatal intensive care unit(NICU)and their psychological resilience and social support of the family members.Methods:The family members of 103 premature infants in NICU were selected as the research subjects from July 2020 to July 2023.The general data,the disease uncertainty,the psychological resilience and the social support status of these family members were investigated.Pearson correlation coefficient was used to analyze the correlation between the disease uncertainty of the family members of the premature infants in NICU and the psychological resilience and social support of the family members.Binary Logistic regression was performed to analyze the influencing factors of the disease uncertainty of the family members of the premature infants in NICU.Results:The low,medium and high levels of the disease uncertainty of the family members of the premature infants in NICU were 16.5%,39.8%and 43.7%,respectively.The total scores of the disease uncertainty,the resilience and the social support of the family members were 83.91±12.12 points,67.24±9.69 points and 26.37±4.43 points,respectively.The scores of all dimensions and the total score of the psychological resilience and social support scale of the family members with low level of the disease uncertainty were significantly higher than those of the family members with medium and high level of the disease uncertainty.The disease uncertainty of the family members was negatively correlated with their psychological resilience and social support(P<0.05).Regression analysis showed that the low levels of education and family income,the severe illness of premature infants,and the low psychological resilience and social support of the family members of the premature infants in NICU were the influencing factors of their disease uncertainty(all P<0.05).Conclusion:In this survey,the disease uncertainty of the family members of the premature infants in NICU is in high level,and which is closely related to the psychological resilience and social support of the family members.It is suggested that the society should pay attention to the mental health problems of the family members of the high-risk premature infants,and the psychological resilience and social support of the family members should be improved to help to reduce their disease uncertainty.
