摘要
通过整理分析张仲景《伤寒杂病论》中涉及针灸和脉诊相关的条文,从病证层面、营卫层面和八纲层面概括仲景的针灸脉证思想。基于仲景和众学者对切脉针灸的阐发和论述,结合笔者对《伤寒杂病论》的学习体会,探讨针灸前先诊脉的必要性,并构建“脉-证-经-穴”针灸诊疗模式,强调先诊脉再针灸的“辨证先行”思想,以期帮助针灸医师在临床上更好地运用针灸治疗疾病。
By systematically analyzing the passages related to acupuncture and moxibustion and pulse diagnosis in ZHANG Zhongjing's Shang Han Za Bing Lun(Treatise on Cold Damage and Miscellaneous Diseases),this study summarizes ZHANG's acupuncture and moxibustion-pulse diagnostic philosophy across three dimensions:disease syndromes,the nutritive and defensive qi,and the eight principles.Building on ZHANG's theories and scholarly interpretations of pulse-guided acupuncture and moxibustion,combined with the authors'insights from studying Shang Han Za Bing Lun,we explore the necessity of pulse diagnosis prior to acupuncture and moxibustion and propose a"pulse-syndrome-meridian-acupoint"diagnostic and treatment model.This model emphasizes the principle of"prioritizing syndrome differentiation through pulse assessment"to guide clinical acupuncture and moxibustion practice,aiming to enhance therapeutic precision and effects.
