摘要
目的探讨乳果糖联合双歧杆菌乳杆菌三联活菌片对便秘型肠易激综合征(C-IBS)患者的症状改善效果及肠道菌群水平的影响。方法采用奇偶分组法将168例C-IBS患者分为观察组(n=84)、对照组(n=84)。对照组给予双歧杆菌乳杆菌三联活菌片治疗,观察组则在对照组基础上联合乳果糖治疗。对比两组疗效、症状改善状况、肠道菌群水平、胃动素(MLT)、血管活性肠肽(VIP)、不良反应发生情况。结果治疗后,观察组治疗总有效率为96.43%,高于对照组的86.90%(P<0.05)。治疗后,两组腹痛、腹胀、大便频率、大便性状评分均低于本组治疗前,观察组腹痛评分(0.98±0.15)分、腹胀评分(1.12±0.15)分、大便频率评分(0.87±0.11)分、大便性状评分(0.97±0.11)分均低于对照组的(1.78±0.24)、(1.58±0.27)、(1.16±0.25)、(1.23±0.34)分(P<0.05)。治疗后,两组双歧杆菌、乳酸杆菌均高于本组治疗前,肠杆菌、拟杆菌均低于本组治疗前,观察组双歧杆菌(9.78±1.13)logCFU/g、乳酸杆菌(9.35±1.51)logCFU/g均高于对照组的(8.78±1.04)、(8.48±1.15)logCFU/g,肠杆菌(7.57±1.48)logCFU/g、拟杆菌(7.76±1.45)logCFU/g均低于对照组的(8.43±1.55)、(8.57±1.56)logCFU/g(P<0.05)。治疗后,两组MLT、VIP均低于本组治疗前,观察组MLT(30.67±3.78)ng/L、VIP(13.76±2.56)ng/L均低于对照组的(34.85±4.09)、(15.65±3.11)ng/L(P<0.05)。治疗期间,观察组不良反应发生率为3.57%(3/84),对照组不良反应发生率为2.38%(2/84);两组不良反应发生率对比无明显差异(χ^(2)=0.206,P=0.650>0.05)。结论乳果糖联合双歧杆菌乳杆菌三联活菌片治疗C-IBS的效果较好,可缓解临床症状,利于MLT和肠道菌群水平的改善,用药价值较高。
Objective To explore the effects of lactulose combined with live combined bifidobacterium and lactobacillus tablets on symptom improvement and intestinal flora level of constipation-predominant irritable bowel syndrome(C-IBS).Methods According to odd-even grouping method,168 patients with C-IBS were divided into observation group(n=84)and control group(n=84).The control group was treated with live combined bifidobacterium and lactobacillus tablets,and the observation group was treated with lactulose on the basis of the control group.The curative effect,improvement of symptoms,levels of intestinal floras,motilin(MLT),vasoactive intestinal peptide(VIP),occurrence of adverse reactions were compared between the two groups.Results After treatment,the total effective rate of the observation group was 96.43%,which was higher than 86.90%of the control group(P<0.05).After treatment,the scores of abdominal pain,abdominal distension,stool frequency and stool traits in both groups were lower than those before treatment;in the observation group,the abdominal pain score was(0.98±0.15)points,the score of abdominal distension was(1.12±0.15)points,the score of stool frequency was(0.87±0.11)points,and the score of stool trait was(0.97±0.11)points,which were lower than(1.78±0.24),(1.58±0.27),(1.16±0.25),and(1.23±0.34)points in the control group(P<0.05).After treatment,the counts of Bifidobacterium and Lactobacillus in both groups were higher than those before treatment,while the counts of Enterobacter and Bacteroides were lower than those before treatment;the counts of Bifidobacterium and Lactobacillus were(9.78±1.13)and(9.35±1.51)logCFU/g in the observation group,which were higher than(8.78±1.04)and(8.48±1.15)logCFU/g in the control group;the counts of Enterobacter and Bacteroides were(7.57±1.48)and(7.76±1.45)logCFU/g in the observation group,which were lower than(8.43±1.55)and(8.57±1.56)logCFU/g in the control group(P<0.05).After treatment,MLT and VIP in both groups were lower than those before treatment;the observation group had MLT of(30.67±3.78)ng/L and VIP of(13.76±2.56)ng/L,which were lower than(34.85±4.09)and(15.65±3.11)ng/L in the control group(P<0.05).During treatment,the incidence of adverse reactions was 3.57%(3/84)in the observation group and 2.38%(2/84)in the control group;there was no significant difference in the incidence of adverse reactions between the two groups(χ2=0.206,P=0.650>0.05).Conclusion Curative effect of lactulose combined with live combined bifidobacterium and lactobacillus tablets is good in C-IBS patients,which can alleviate clinical symptoms and is beneficial to improve levels of MLT and intestinal floras,with high medication value.
