营养支持对肺结核患者营养状况和免疫功能的影响 被引量:1

Effects of nutritional support on nutritional status and immune function in patients with pulmonary tuberculosis

摘要 目的 探讨营养支持对肺结核患者营养状况和免疫功能的影响。方法 选取2023年1月至12月在广州市胸科医院住院的120例肺结核患者,依据随机数字表法将其分为常规组(n=60)和营养组(n=60)。常规组采用常规治疗,营养组在常规组治疗基础上给予营养支持。比较两组的治疗效果。结果 治疗前,两组的体质量指数(BMI)、总蛋白(TP)、白蛋白(ALB)、前白蛋白(PA)、转铁蛋白(TRF)及血红蛋白(Hb)水平比较,差异无统计学意义(P>0.05);治疗后,两组的BMI、PA及Hb水平高于治疗前,差异具有统计学意义(P<0.05);治疗后,营养组的PA、Hb水平高于常规组,差异具有统计学意义(P<0.05)。治疗前,两组的淋巴细胞计数(LYM)、中性粒细胞与淋巴细胞比值(NLR)及血小板与淋巴细胞比值(PLR)比较,差异无统计学意义(P>0.05);治疗后,两组的LYM均高于治疗前,NLR、PLR均低于治疗前,差异具有统计学意义(P<0.05);治疗后,营养组的LYM高于常规组,NLR、PLR低于常规组,差异具有统计学意义(P<0.05)。营养组的痰菌改善率为73.33%,高于常规组的53.33%,差异具有统计学意义(P<0.05)。结论 营养支持可改善肺结核患者的营养状况和免疫功能,有利于提高临床疗效与预后,值得临床推广。 Objective To investigate the effects of nutritional support on nutritional status and immune function in patients with pulmonary tuberculosis.Methods A total of 120 patients with pulmonary tuberculosis who were hospitalized in Guangzhou Chest Hospital from January to December 2023 were selected and divided into conventional group(n=60)and nutrition group(n=60)according to the random number table method.The conventional group was treated with routine treatment,and the nutrition group was given nutritional support on the basis of the conventional group.The therapeutic effects of the two groups were compared.Results Before treatment,there were no statistically significant differences in the body mass index(BMI),total protein(TP),albumin(ALB),prealbumin(PA),transferrin(TRF)and hemoglobin(Hb)levels between the two groups(P>0.05);after treatment,the BMI,PA and Hb levels in the two groups were higher than those before treatment,and the differences were statistically significant(P<0.05);after treatment,the levels of PA and Hb in the nutrition group were higher than those in the conventional group,and the differences were statistically significant(P<0.05).Before treatment,there were no statistically significant differences in lymphocyte count(LYM),peripheral blood neutrophil to lymphocyte ratio(NLR)and platelet to lymphocyte ratio(PLR)between the two groups(P>0.05);after treatment,the LYM in both groups was higher than that before treatment,and NLR and PLR were lower than those before treatment,the differences were statistically significant(P<0.05);after treatment,the LYM of the nutrition group was higher than that of the conventional group,and the NLR and PLR were lower than those of the conventional group,the differences were statistically significant(P<0.05).The improvement rate of sputum bacteria in the nutrition group was 73.33%,which was higher than 53.33%in the conventional group,and the difference was statistically significant(P<0.05).Conclusion Nutritional support can improve the nutritional status and immune function of patients with pulmonary tuberculosis,which is conducive to improving clinical efficacy and prognosis,and is worthy of clinical promotion.
