摘要
目的:探究输尿管软镜碎石术治疗上尿路结石的临床效果。方法:选取2023年9月—2024年9月合肥市桐梓县中医医院肛肠泌尿外科收治的上尿路结石患者150例开展前瞻分析,按随机数字表法分组,参照组75例患者采用经皮肾镜取石术,试验组75例采用输尿管软镜碎石术。对比手术相关指标、结石清除率、疼痛程度及并发症发生情况。结果:试验组手术时间、激光碎石时间、术后首次下床时间、术后住院时间均短于参照组,出血量少于参照组(P<0.05);试验组一次性结石清除率、术后4周结石清除率高于参照组,二次碎石术率低于参照组(P<0.05);术后当天、术后3 d,两组数字模拟疼痛评分量表评分降低,试验组低于参照组(P<0.05);试验组并发症发生率低于参照组(P=0.009)。结论:应用输尿管软镜碎石术治疗上尿路结石,可提高结石清除率,减轻患者疼痛,促进其术后更快恢复,缩短激光碎石时间,且并发症少。
Objective:To explore the clinical effect of ureteroscopic lithotripsy in the treatment of upper urinary tract stones.Methods:A prospective analysis was performed on 150 patients with upper urinary tract stones admitted to the Department of Anorectal Urology of Zunyi Tongzi County Traditional Chinese Medicine Hospital from September 2023 to September 2024.The patients were divided into groups according to the random number table method.75 patients in the reference group underwent percutaneous nephrolithotomy,and 75 patients in the experimental group underwent ureteroscopic lithotripsy.The surgical related indicators,stone clearance rate,pain degree and complications were compared.Results:The operation time,laser lithotripsy time,first time to get out of bed after surgery,and postoperative hospitalization time of the experimental group were shorter than those of the reference group,and the amount of bleeding was less than that of the reference group(P<0.05).The one-time stone clearance rate and the stone clearance rate of 4 weeks after surgery in the experimental group were higher than those in the reference group,and the secondary lithotripsy rate was lower than that in the reference group(P<0.05).On the day after surgery and 3 days after surgery,the scores of the digital analog pain rating scale in the two groups decreased,and the scores in the experimental group were lower than those in the reference group(P<0.05).The incidence of complications in the experimental group was lower than that in the reference group(P=0.009).Conclusion:The application of ureteroscopic lithotripsy in the treatment of upper urinary tract stones can improve the stone clearance rate,reduce the pain perception of patients,promote faster recovery after surgery,shorten the laser lithotripsy time,and have fewer complications.
