摘要
目的:了解原发性肝癌患者的凝血功能障碍情况,并探讨其临床意义。方法。对40例原发性肝癌患者进行凝血酶原时间(PT)、活化部份凝血酶原时间(APTT)、纤维蛋白原(FIB)、凝血酶时间(TT)四项凝血指标和血小板计数(PLT),平均血小板体积(MPV)、血小板压积(PCT)、血小板分布宽度(PDW)四项血小板参数进行检测。结果:原发性肝癌组与对照组相比,PT、APTT、TT均显著延长(P<O.01),FIB明显降低(]<0.05),PLT、PCT显著降低(P<0.01),MPV、PDW显著升高(P<0.01):肝癌并发症组与无并发症组相比.PT、APTT均显著延长(P<0.01.P<O.05)、FIB显著降低(P<0.01),MPV、PDW明显升高(P<0.05),TT、PLT、PCT差别无显著性意义;原发性肝癌转移组与无转移组相比,PT、APTT明显延长(P<0.05),FIB显著降低(P<0.05),MPV、PDW明显升高(P<0.05),TT、PLT、PCT差别无显著性意义。结论;原发性肝癌患者可出现凝血功能障碍和血小板参数变化,对评估肝癌肝损害的程度、监测病情变化,指导临床治疗及其预后具有一定的应用价值。
Objective: To investigate the clinical significance of coagulation parameters and platelet parameters in primary hepatic carcinoma patients. Methods: To examine the levels of Prothrombin time(PT), activated partial thromboplastin time(APTT), fib-rinogen(FIB), thrombin time(TT), Platelet count(PLT), mean platelet volume(MPV), plateletcrit(PCT), platelet distribution width (PDW) in 40 patients with primary hepatic carcinoma. Results: The levels of PT, APTT, TT were significantly prolonged , and the MPV, PDW were increased obviously, whereas the FIB, PLT, PCT were markedly reduced in patients with primary hepatic carcinoma than those in controls. Moreover, PT, APTT notably prolonged, MPV, PDW obviously increased, but FIB reduced significantly in patients comparing complication of hepatic carcinoma with those simplication. TT, PLT, PCT have no changes between complication group and simplication group. Comparing metastasis carcinoma with preinvasive carcinoma, PT, APTT prolonged, MPV, PDW obviously increased, and FIB reduced significantly. TT, PLT, PCT also have no changes between metastasis carcinoma and preinvasive carcinoma. Conclusion: The results showed the decrease of coagulability and changes of platelet parameters in primary hepatic carcinoma patients. These markers are useful to evaluate liver damage, predict complication and metastasis .estimate prognosis.
