摘要
目的探讨肝病患者红细胞、血小板参数、凝血功能指标的变化及临床意义。方法采用日本Sysmex-K-4500血液分析仪和法国STA-R全自动凝血仪对我院200例肝病患者和60例健康体检者进行红细胞、血小板参数、凝血功能指标的检测分析。结果急性肝炎、慢性肝炎、肝硬化、肝癌组红细胞数(RBC)、血红蛋白浓度(HGB)、血细胞比容(PCV)、血小板数(PLT)、纤维蛋白原(FIB)均低于正常对照组,有显著性差异(P<0.01);红细胞平均容积(MCV)、红细胞平均血红蛋白含量(MCH)、红细胞平均血红蛋白浓度(MCHC)、红细胞体积分布宽度(RDW%)、血小板平均体积(MPV)、血小板体积分布宽度(PDW)、大血小板比率(P-LCR%)、凝血酶原时间(PT)、凝血酶原时间国际标准比值(PT-INR)、活化部分凝血活酶时间(APTT)、凝血酶时间(TT)均高于正常对照组,有显著性差异(P<0.05或P<0.01)。结论肝病患者红细胞、血小板参数、凝血功能指标检测结果均有所变化,且变化的程度与病情的进展和预后密切相关,可作为肝病患者病情进展及严重程度的重要参考指标,在指导临床治疗有一定应用价值。
Objective To observe the variations of levels of red blood cell, blood platelets, blood coagulation in hpatopathy patients and their clinical significance. Methods The levels of red blood cell, blood platelet, blood coagulation in 200 hepatopathy patients and 60 healthy people were determined by Sysmex - K - 4500 and STA - R. Results The levels of red blood cell count （RBC）, hemoglobin concentration （HGB）, packed cell volume （PCV）, platelet count （PLT） ,fibrinogen（FIB） were obviously reduced in hepatopathy patients with acute hepatitis, chronic hepatitis, hepatocirrhosis, liver cancer comparing with the ones in the control group. Moreover, the value of mean corpuscular volume （ MCV）, mean corpuscular hemoglobin （MCH） ,mean corpuscular hemoglobin concentration （ MCHC）, red blood cell volume distribution width （ RDW% ）, mean platelet volume （MPV）, platelet distribution width （PDW）, platelet large cell ratio （ P - LCR% ）, prothrombin time （ PT ）, prothrombin time international normalization ratio（ PT -INR）, activated partial thromboplasting time（APTT） ,thrombin time（TT） were obviously increased in hepatopathy patients comparing with the ones in the control group （P 〈 0.05 or P 〈 0.01 ）. Conclusion There were significant changes of the value of red blood cell, blood platelet and blood coagulation parameters. Furthermore, the changes of the levels of red blood cell, blood platelet, blood coagulation parameters are not only closedly associted with the progression and prognosis of the disease but also are the significant reference indicators for diagnosis of progression and severity of the disease and guiding clinical treatment.
