摘要
目的:研究脉冲冲洗器预防经椎间孔入路腰椎椎间融合术(TLIF)患者术口感染的临床效果。方法:选取2022年1月—2023年12月于云浮市新兴县人民医院接受TLIF治疗的腰椎退变性疾病患者62例,以信封随机法分成观察组与对照组,各31例。两组均进行TLIF,观察组应用脉冲冲洗器冲洗切口,对照组应用常规人工倾倒冲洗法冲洗切口。比较两组围术期指标、C反应蛋白(CRP)水平、体温、并发症发生率、视觉模拟评分法评分、切口拆线时间。结果:两组手术时间、拔引流管时间对比,差异无统计学意义(P>0.05);观察组冲洗时间短于对照组,术后引流量高于对照组(P<0.001)。术前,两组CRP水平、体温比较,差异无统计学意义(P>0.05);术后1、3、5d,两组CRP水平升高,术后3、5d,观察组低于对照组(P<0.05);术后3、5d,两组体温升高,观察组低于对照组(P<0.05)。观察组术后并发症总发生率低于对照组(P=0.045)。观察组视觉模拟评分法评分低于对照组,切口拆线时间早于对照组(P<0.001)。结论:脉冲冲洗器应用于TLIF可有效缩短冲洗时间,降低引流液排出难度,减轻炎症,有效预防术后感染。
Objective:To study the elinical effect of pulse irigator in preventing surgical port infection in patients undergoing lum-bar interbody fusion(TLIF)via transforaminal approach.Methods:Sixty-two patients with lumbar degenerative diseases who under-went TLIF at Yunfu Xinxing County People's Hospital from January 2022 to December 2023 were selected and divided into an ob-servation group and a control group,each with thirty-one cases,by envelope randomization method.TLIF was performed in both groups,and the observation group applied the pulse flosser to flush the incision,while the control group applied the conventional manual dumping and flushing method to flush the incision.The perioperative indexes,C-reactive protein(CRP)level,body tem-perature,complication rate,visual analogue scoring method score,and incision removal time were compared between the two groups.Results:Comparison of operation time and drain removal time between the two groups showed no statistically significant difference(P>0.05);the observation group had shorter flushing time than the control group,and the amount of postoperative drain-age was higher than that of the control group(P<0.001).Preoperatively,comparison of CRP level and body temperature between the two groups showed no statistically significant difference(P>0.05);at 1,3 and 5 d postoperatively,the CRP levels of the two groups increased,and at 3 and 5 d postoperatively,the observation group was lower than the control group(P<0.05);at 3 and 5 d postopera-tively,the body temperaturesofthe twogroups increased,and the observation group was lowerthanthe control group(P<0.05).The total in-cidence of postoperative complications in the observation group were lower than that in the control group(P=0.045).The visual ana-logue scoring method score of the observation group was lower than that of the control group,and the incision removal time was ear-lier than that of the control group(P<0.001).Conclusion:The application of pulse flusher in TLIF can efectively shorten the flush-ing time,reduce the difeculty of drainage fluid discharge,reduce inflammation,and efectively prevent postoperative infection.
