摘要
为实现病有所医的民生保障目标,我国将推动非基本公共医疗服务普惠化,而现实中一些已经用于临床的新技术对普通民众仍可望而不可及。伴随时代进步,基本与非基本公共服务的边界发生了变化,公益性成为界定各方权责和制定卫生政策的基础。文章通过分析医学服务公益性分类及僵化认识,发现二代测序服务公益性被忽视,由可及性衍生出公平性,间接损害民众健康权,加剧了看病难等系列问题。事实上,该技术用于产前筛查、疑难杂症、肿瘤、严重药物不良反应等项目时公益效应明显。因此公益性不应成为其应用桎梏。通过定价、医保和监督改革,对公益项目可实现普惠化应用,由此探索国内新诊疗技术跨越式发展路径。
In order to ensure people s livelihood of having access to medical services,China will promote inclusive non-basic public medical services,while some new technologies that have been used in clinical practice are still out of reach for ordinary people.With the social progress,the boundary between basic and non-basic public services has changed,and public welfare has become the basis for defining the rights and responsibilities of various parties and formulating health policies.By analyzing the classification and rigid understanding of the public welfare of medical services,the paper finds that the public welfare of next-generation sequencing services is ignored,and equity is derived from accessibility,which indirectly damages people s right to health and aggravates a series of problems such as the difficulty to obtain medical services.In fact,the public benefits of this technology are obvious when it is used in prenatal screening,complicated diseases,tumors,serious adverse drug reactions and so on.Therefore,public welfare should not be the fetter of its application.Through the reform of pricing,medical insurance and supervision,the inclusive application of public welfare projects can be realized,and the path of leapfrog development of domestic new diagnosis and treatment technologies can be explored.
