摘要
目的 探讨系统性红斑狼疮(SLE)女性感染宫颈人乳头瘤病毒(HPV)的基因型及危险因素。方法 对73例SLE女性和2 251例健康体检女性进行宫颈HPV检测,对比2组女性感染HPV亚群特征,并分析感染HPV的SLE女性用药、疾病病程、免疫学特征、器官受累数目及SLE-DAI2000等特征。结果 SLE女性与健康体检女性比较,HPV感染总数、HPV多重感染差异有统计学意义(P<0.05),而2组HPV单一感染差异无统计学意义(P=0.059)。HPV(+)SLE和HPV(-)SLE比较,SLE病程和环磷酰胺使用差异有统计学意义(P<0.05),而抗核抗体滴度、补体C3、补体C4、主要器官受累数目、疾病活动度、糖皮质激素及除环磷酰胺以外其他免疫抑制剂使用差异无统计学意义(P>0.05)。结论 SLE女性更易感染HPV,且多见多重感染,随着SLE病程的延长和环磷酰胺的使用SLE女性感染HPV机会也增加。临床中应重视SLE女性常规妇科监测及做好对HPV感染的预防。
Objective This paper aims to investigate the genotypes and their risk factors of cervical human papillomavirus(HPV) in women with systemic lupus erythematosus(SLE). Methods A total of 73 SLE women and 2 251 healthy women were conducted for the test of cervical HPV, and the characteristics of HPV subtypes in the two groups were compared. The use of medication, disease course, immunological characteristics, the number of organs involvement and SLE-DAI2000 on HPV infection in SLE women were analyzed. Results There was statistical significance on the differences in total HPV infection and HPV complex infection between SLE women and healthy women(P<0.05), while there was no statistical significance on the differences in HPV single infection between the two groups(P=0.059). There was statistical significance on the differences between SLE course and cyclophosphamide use between HPV(+) SLE and HPV(-) SLE(P<0.05), while there was no statistical significance on the differences in anti-nuclear antibody titers, complement C3, complement C4, the number of organs involvement, disease activity, glucocorticoids and other immunosuppressive agents except cyclophosphamide use(P>0.05). Conclusion SLE women are prone to be infected with HPV, and multiple infections are more common. With the prolongation of SLE course and the use of cyclophosphamide, the possibility of HPV infection in SLE women also increases. Routine gynecological monitoring and HPV prevention measures should be carried out on SLE women.
