摘要
目的分析中国预防接种后心因性反应发生特征。方法通过中国疑似预防接种异常反应(Adverse event following immunization,AEFI)信息管理系统收集2010-2019年心因性反应报告个案,描述性分析心因性反应报告发生率、在AEFI中比例等指标。结果中国2010-2019年共报告心因性反应1162例,在AEFI中比例为0.07%,报告发生率为0.22/100万剂。男、女性心因性反应在AEFI中比例分别为0.05%、0.10%,≤6岁、7-17岁、≥18岁分别为0.02%、1.84%、1.40%。甲型H1N1流感疫苗、人乳头瘤病毒疫苗、麻疹疫苗的心因性反应在AEFI中比例较高,分别为4.01%、2.55%、0.61%。80.45%(683/849)的心因性反应发生在接种疫苗后30min内。共报告聚集性心因性反应10起,均发生在中小学生群体性预防接种活动中。结论中国2010-2019年预防接种后心因性反应总体发生率极低,女性、青少年或大年龄人群发生风险相对较高;群体性接种活动中需做好聚集性心因性反应的防范和应对。
Objective To analyze psychogenic reactions following immunization in China.Methods We obtained information on psychogenic reactions following immunization that were reported to the China Adverse Event Following Immunization(AEFI)Information Management System in 2010-2019 to determine incidences of psychogenic reactions and proportions of AEFIs that were psychogenic reactions.Results A total of 1162 psychogenic reactions were reported in 2010-2019 in China,amounting to 0.07%of all AEFI reports and a reported incidence of 0.22 cases per million doses administered.The percent of reported AEFIs that were psychogenic reactions were 0.05%among males and 0.10%among females;by age,the percents of AEFIs that were psychogenic reaction were 0.02%,1.84%,and 1.40%for≤6-,7-17-,and≥18-year-olds;and by vaccine,were 4.01%,2.55%,and 0.61%for influenza A(H1N1)vaccine,human papillomavirus vaccine,and measles-containing vaccine;80.45%(683/849)of psychogenic reactions occurred within 30 minutes after vaccination.Ten psychogenic reaction clusters were reported and occurred in mass vaccination activities of primary and middle school students.Conclusions The overall incidence of psychogenic reactions following immunization was extremely low in 2010-2019 in China;risk was highest among females,adolescents,and older people.We should enhance prevention of and response to psychogenic reaction clusters during mass vaccination activities.
