摘要
目的观察八正化石胶囊联合坦洛新辅助体外冲击波碎石术(ESWL)治疗输尿管壁内段结石的临床效果。方法选择2020年1月至2021年12月河北省沧州中西医结合医院泌尿外科收治的输尿管壁内段结石患者93例,按照随机数字表法分为2组,2组均接受ESWL治疗,对照组(46例)术后予盐酸坦洛新缓释片治疗,治疗组(47例)术后在对照组的基础上加八正化石胶囊治疗,2组治疗时间均为15 d。比较2组治疗前后中医症状评分、肝肾功能变化,以及结石排净率、结石排净时间、第2次碎石率、抗菌药物使用率、抗菌药物使用天数、肾绞痛发作率、止痛药物使用率及不良事件发生情况。结果治疗后,2组中医症状各项评分及总分均较本组治疗前降低(P<0.05),且治疗组均低于对照组(P<0.05)。治疗组术后第5、10、15 d结石排净率均高于对照组(P<0.05),结石排净时间和抗菌药物使用天数短于对照组(P<0.05),第2次碎石率和止痛药物使用率低于对照组(P<0.05)。2组抗菌药物使用率和肾绞痛发作率比较差异无统计学意义(P>0.05)。治疗后,2组肝功能指标总胆红素(TBiL)、天冬氨酸氨基转移酶(AST)、丙氨酸氨基转移酶(ALT)水平及肾功能指标血肌酐(SCr)和尿素氮(BUN)水平均较本组治疗前降低(P<0.05),且治疗组均低于对照组(P<0.05)。2组不良事件总发生率比较差异无统计学意义(P>0.05)。结论八正化石胶囊联合坦洛新辅助ESWL治疗输尿管壁内段结石可改善患者临床症状,促进碎石排出,降低止痛药物使用率,改善肝肾功能,且安全可靠。
Objective To observe the clinical effect of Bazheng Huashi capsules combined with tamsulosin tablets and extracorporeal shock wave lithotripsy(ESWL)for distal ureteral calculi(DUC).Methods From January 2020 to December 2021,totally 93 DUC patients who were admitted to the Department of Urology,Cangzhou Hospital of Integrated Traditional Chinese and Western Medicine were randomly assigned to receive either ESWL+tamsulosin tablets(the control group,n=46)or ESWL+tamsulosin tablets+Bazheng Huashi capsules(the treatment group,n=47).Treated for 12 weeks,the aim was to compare traditional Chinese medicine(TCM)symptom score,liver and kidney function,stone clearance rate,the time to stone elimination,the second-stage lithotripsy rate,antimicrobial use rate,antibiotic use duration,frequency of renal colic pain,analgesic use,adverse events.Results After treatment,TCM symptom scores in the both groups were decreased(P<0.05),which were decreased in the treatment group compared with the control group(P<0.05);On the 5th,10th,and 15th day after operation,the stone clearance rate in the treatment group was higher than that in the control group(P<0.05),and the time to stone elimination and antibiotic use duration were shorter than those in the control group(P<0.05).The second-stage lithotripsy rate and analgesic use rate were lower than those in the control group(P<0.05).The difference was not statistically significant in the ratio of antimicrobial use and renal colic pain between groups(P>0.05).The liver function indexes(total bilirubin[TBil],aspartate aminotransferase[AST],alanine aminopeptidase[ALT]),renal function indexes(serum creatinine[SCr],blood urea nitrogen[BUN])in the both groups were decreased after treatment,which were lower in the treatment group than in the control group(P<0.05).None of the total incidence of adverse events differed significantly between groups(P>0.05).Conclusion For DUC patients,the three combinations can improve clinical symptoms,promote the stone removal,reduce the rate of analgesic use,improve liver and kidney functions,with safety and reliability.
