摘要
目的:评价甲沟重建联合甲内卷矫正术治疗嵌甲型甲沟炎的临床疗效。方法:选择2019年12月至2021年10月在我科就诊的重度嵌甲型甲沟炎患者48例,均采用甲沟重建联合甲内卷矫正术治疗。手术要点:①楔形切除增生的肉芽组织缩小甲襞,通过新的缝合方法使甲襞侧组织与甲床同一水平,构建正常甲沟;②橡胶片固定在甲缘下以钝化甲边缘;③用橡皮筋牵拉甲缘下橡胶片以抬高甲侧缘以纠正甲内卷畸形。结果:48例患者(81个拇趾)全部治愈,甲形态正常且美观,随访6个月无复发。结论:甲沟重建联合甲内卷矫正术疗效好。
Objective:To assess the efficacy of nail groove reconstruction combined with correction of the nail curl deformity on ingrown paronychia.Methods:48 patients with severe ingrown paronychia in our department from December 2019 to October 2021 were treated with nail groove reconstruction and combined with correction of the nail curl deformity.The key techniques of the operation included wedge-shaped excision of proliferative granulation tissue to reduce nail fold,then the side tissue of nail fold and nail bed were made at the same level through our suture method;two small rectangular rubber sheets are respectively fixed under the both edges of the nail to passivate the nail edge;two rubber bands was used to pull the two rubber sheets under the nail margin to raise the bilateral margins of the nail for correction the nail curl deformity.Results:All 48 patients(81 big toes)were cured and there was no recurrence after 6 months of follow-up.Conclusion:The efficacy of nail groove reconstruction combined with nail curl for ingrown paronychia is good.
