DWI与血清PCSK9、IL-18在乳腺癌中的诊断效果 被引量:1

摘要 目的:评估弥散加权成像(DWI)与血清前蛋白转化酶枯草溶菌素9(PCSK9)、白细胞介素18(IL-18)在乳腺癌中的诊断价值。方法:前瞻性选取我院2020年4月—2022年4月收入的疑似乳腺癌患者100例,均行DWI及病理检查,根据病理检查结果分为恶性肿瘤组(62例)、良性肿瘤组(38例),测两组患者血清PCSK9、IL-18表达水平。比较恶性肿瘤组与良性肿瘤组患者DWI检查下ADC值差异以及血清PCSK9、IL-18水平差异。ROC曲线分析DWI与血清PCSK9、IL-18单独及联合诊断乳腺癌的价值。结果:DWI检查下b=400s/mm^(2)时ADC值明显高于b=800s/mm^(2)时,且b=400s/mm^(2)、800s/mm^(2)时良性肿瘤组ADC值均高于恶性肿瘤组,差异有统计学意义(P<0.05)。恶性肿瘤组血清PCSK9、IL-18水平显著高于良性肿瘤组,差异有统计学意义(P<0.05)。Logistic回归分析显示ADC值、血清PCSK9、IL-18水平是影响乳腺癌患病的危险因素(P<0.05)。ADC值(b=400s/mm^(2))、血清PCSK9、IL-18水平单独诊断乳腺癌曲线下面积(AUC)分别为0.770、0.914、0.881,联合诊断乳腺癌AUC为0.983。结论:DWI与血清PCSK9、IL-18诊断乳腺癌均有一定价值,且三者联合时诊断精准性更高。 Objective:To evaluate the diagnostic value of diffusion weighted imaging(DWI),serum preprotein invertase subtilisin 9(PCSK9)and interleukin 18(IL-18)in breast cancer.Methods:100 suspected breast cancer patients from April 2020 to April 2022 in our hospital were prospectively selected for DWI and pathological examination.According to the pathological examination results,they were divided into malignant tumor group(62 cases)and benign tumor group(38 cases).The expression levels of serum PCSK9 and IL-18 in the two groups were measured.The differences of ADC value under DWI and the levels of serum PCSK9 and IL-18 between malignant tumor group and benign tumor group were compared.ROC curve analysis of DWI and serum PCSK9,IL-18 in the diagnosis of breast cancer.Results:Under DWI examination,the ADC value of b=400s/mm^(2) was significantly higher than that of b=800s/mm^(2),and the ADC value of benign tumor group was higher than that of malignant tumor group at b=400s/mm^(2) and 800s/mm^(2)(P<0.05).The levels of serum PCSK9 and IL-18 in malignant tumor group were significantly higher than those in benign tumor group(P<0.05).Logistic regression analysis showed that ADC value,serum PCSK9 and IL-18 levels were the risk factors of breast cancer(P<0.05).The AUC of ADC value(b=400s/mm^(2)),serum PCSK9 and IL-18 levels were 0.770,0.914 and 0.881 for the diagnosis of breast cancer alone,and the combined diagnosis of breast cancer was 0.983.Conclusion:DWI,serum PCSK9 and IL-18 have certain value in the diagnosis of breast cancer,and the diagnostic accuracy is higher when the three are combined.
