摘要
目的:探讨MRI动态增强时间-信号强度曲线(TIC)联合弥散加权成像(DWI)对乳腺良恶性病变的鉴别价值。方法:收集经手术病理证实的40例(41个病灶)乳腺病变患者的MRT1WI、T2WI、DWI(b值=800 s/mm2)和MRI动态增强扫描资料,对病灶信号特征、ADC值和增强TIC进行回顾性分析。结果:40例41个病灶中,TICⅠ型曲线,良性10个,恶性0个;Ⅱ型曲线,良性10个,恶性4个;Ⅲ型曲线,良性3个,恶性15个;Ⅳ型曲线,2个病灶均为良性病变。病灶的ADC值>1.2×10-3mm2/s的24个(其中良性21个,恶性3个);ADC值<1.2×10-3mm2/s的18个(其中恶性16个,良性2个)。结论:MRI动态增强联合DWI对乳腺病变的诊断有较高的敏感性和特异性,增强TIC类型和ADC值对乳腺病变的良恶性鉴别具有重要价值。
Objective: To explore the diagnostic value of dynamic contrast- enhanced MRI time- signal intensity curve (TIC) combined diffusion- weighted imaging (DWI) in distinguishing the benign and malignant breast lesions. Methods: Forty cases(41 lesions) with pathologically confirmed breast lesions had MRI T1WI, T2WI, DWI (b value = 800 s/mm2)and dynamic contrast- enhanced MR scan. The signal intensity, apparent diffusion coefficient (ADC) values , enhanced TIC of the lesions were retrospectively analyzed. Results The types of TIC in all 41 lesions(40 cases) included type I curve(10 benign and malignant 0); Ⅱ curve(10 benign, 4 malignant) ;Ⅲ curve(3 benign, malignant 15) ; IV curve(2 benign lesions). The ADC values of 24 lesions〉 1.2 x 10-Smm2/s (21 benign, 3 malignant ) ; ADC values of 18 lesions 〈 1.22 x 10 - 3 mm2/s (16 malignant and 2 benign ). Conclusion: Dynamic contrast - enhanced MRI TIC combined DWI has high sensitivity and specificity in diagnosing breast lesions. The type of TIC and the value of ADC play important roles in differenti- ating benign and malignant breast lesions.
