摘要
水肿发病,病机复杂。气运不畅、枢机不利是其重要病变机理。气之运动的关键在于枢机,横枢为少阳,调节横向气机;纵枢为脾胃,调节纵向气机。二者在生理和病理上密不可分。水肿的治疗在于治气,故治疗的关键在于调理气之横纵枢机。横枢位于气之出入的必经之地,调横枢旨在疏调少阳,使水湿从里走表,宣散而解,注重通调横枢,疏、和、透三法并用。纵枢位于中焦,为气机升降斡旋之所,调纵枢旨在调理脾胃,恢复气机之升降则水可上腾下达。治疗时应注重补脾胃之虚,以调为法,用药宜轻灵,以防滋腻碍脾。同时须辨证论治,以确定调理横枢或纵枢,或二者兼调,不可拘泥一方。
The pathogenesis of the onset of edema is complex,in which poor Qi movement,and unfavorable pivots are important pathological mechanisms.The key point of the movement of Qi lies in the pivots.The horizontal pivot is Shaoyang,which regulates the horizontal Qi movement.The vertical pivot is the Spleen and Stomach,which regulates the vertical Qi movement.The two are inseparable in physiology and pathology.The treatment of edema lies in the treatment of Qi,so the key point of treatment lies in regulating the horizontal and vertical pivots of Qi.The horizontal pivot is located at the place where the Qi pass in and out.The purpose of adjusting the horizontal pivot is to dredge the Shaoyang,so that the Water and Dampness can travel from the inside to the outside,and eventually get dispersed and dissolved.The adjustment of the horizontal pivot should be paid attention to,and the three methods of dredging,harmonizing,and penetrating can be used.The vertical pivot is located in the Middle Jiao and it is where the Qi movement rises and falls.The purpose of adjusting the vertical pivot is to regulate the Spleen and Stomach.When the Qi movement is restored,the Water can rise or flow down normally.Invigorating the deficiency of the Spleen and Stomach should be paid attention to,by using adjustment method with the medicine of light character to prevent the Spleen from being greasy nourished.At the same time,it is necessary to differentiate and treat according to the syndrome,so as to determine the adjustment of the horizontal pivot or the vertical pivot,or both.Any one side should not be sticked to.
