NFKB1基因多态性与原发性高血压及血清TNF-α的相关性研究 被引量:6

摘要 目的探讨NFKB1启动子区-94ins/del ATTG(rs28362491)基因多态性与原发性高血压(essential hypertension,EH)及TNF-α的关系。方法选择2017年6月至2020年6月金华市人民医院EH患者275例(EH组)和138名血压正常的健康体检者(NT组)为研究对象,利用聚合酶链反应-限制性片段长度多态性技术检测NFKB1基因rs28362491位点等位基因及基因型分布频率,在校正其他EH传统危险因素后,采用logistic回归分析NFKB1-rs28362491基因多态性是否为EH的独立危险因素。采用ELISA法检测并比较两组对象及不同基因型者血清TNF-α水平。结果EH组NFKB1基因rs28362491位点DD基因型频率高于NT组(24.4%比14.5%),II、ID基因型频率低于NT组(43.3%比43.5%,32.3%比42.0%),差异均有统计学意义(均P<0.05)。两组D、I等位基因频率比较(46.0%比36.2%;54.0%比63.8%),差异均有统计学意义(均P<0.05)。两组隐性模型(DD比ID+II)分布差异有统计学意义(P<0.05),但两组显性模型(ID+DD比II)分布差异无统计学意义(P>0.05)。在校正其它危险因素后,DD基因型仍是EH的独立危险因素(OR=1.729,95%CI:1.135~3.926,P<0.05)。与非DD基因型携带者相比,DD基因型携带者血清TNF-α水平明显升高(P<0.05)。结论NFKB1-rs28362491基因多态性与EH相关,DD基因型可能调控TNF-α,成为EH易感性遗传因素。 Objective To investigate the association of NFKB1 promoter region-94ins/del ATTG(rs28362491)gene polymorphism with essential hypertension(EH)and TNFα.Methods From June 2017 to June 2020,275 EH patients and 138 healthy subjects with normal blood pressure(NT)were enrolled in Jinhua People's Hospital.NFKB1 gene rs28362491 allele and genotype distribution frequency were detected by polymerase chain reaction-restriction fragment length polymorphism(PCR-RFLP)assay.The risk factors for EH were determined with logistic regression analysis.Serum TNFαlevel was detected by enzyme-linked immunosorbent assay.Results The DD genotype frequency of NFKB1 gene rs28362491 in EH group was significantly higher than that in NT group(24.4%vs.14.5%,P<0.05)and the genotype frequency of II and ID in EH group was lower than that in NT group(43.3%vs.43.5%,32.3%vs.42.0%),with statistical significance(P<0.05).Comparison of D and I allele frequencies between the two groups(P<0.05).Further analysis showed that there was significant difference in the distribution of recessive model(DD vs.ID+II)between the two groups(P<0.05),but no difference in the distribution of dominant model(ID+DD vs.II)between the two groups(P>0.05).Logistic regression analysis showed that DD genotype was an independent risk factor for essential hypertension(OR=1.729,95%CI:1.135-3.926,P<0.05).Compared with non-DD genotypes,plasma TNFαwas significantly increased in DD genotypes(P<0.05).Conclusion NFKB1-rs28362491 gene polymorphism is associated with EH and DD genotype may regulate TNFαas a genetic factor of EH susceptibility.
