摘要
目的促性腺激素释放激素激动剂(GnRHa)联合桂枝茯苓胶囊在腹腔镜手术中对卵巢巧克力囊肿伴不孕症患者妊娠情况的影响。方法选取2017年1月至2018年10月我院收治的120例卵巢巧克力囊肿伴不孕症患者作为研究对象,根据治疗方式不同将其分为对照组(60例,腹腔镜手术+GnRHa)和观察组(60例,腹腔镜手术+GnRHa+桂枝茯苓胶囊)。比较两组的治疗效果。结果观察组的治疗总有效率、排卵率高于对照组(P<0.05)。治疗前,两组的阻力指数、搏动指数、子宫内膜厚度、卵泡直径比较,差异无统计学意义(P>0.05);治疗后,两组的阻力指数、搏动指数低于治疗前,子宫内膜厚度、卵泡直径大于治疗前,差异具有统计学意义(P<0.05);治疗后,观察组的阻力指数、搏动指数低于对照组,子宫内膜厚度、卵泡直径大于对照组,差异具有统计学意义(P<0.05)。随访2年,观察组的临床妊娠率高于对照组,复发率低于对照组,差异具有统计学意义(P<0.05)。结论腹腔镜手术+GnRHa+桂枝茯苓胶囊治疗卵巢巧克力囊肿伴不孕症患者的效果显著,可提高临床妊娠率,降低复发率,值得临床推广和应用。
Objective To investigate the effect of gonadotropin-releasing hormone agonist(GnRHa)combined with Guizhi Fuling capsule on pregnancy of infertile patients with ovarian chocolate cyst in laparoscopic surgery.Methods A total of120 patients with ovarian chocolate cyst and infertility admitted in our hospital from January 2017 to October 2018 were selected as the research objects.According to different treatment methods,the patients were divided into control group(60cases,laparoscopic surgery+GnRHa)and observation group(60 cases,laparoscopic surgery+GnRHa+Guizhi Fuling capsule).The therapeutic effects of the two groups were compared.Results The total effective rate of treatment and ovulation rate of the observation group were higher than those of the control group(P<0.05).Before treatment,there were no significant differences in resistance index,pulsatility index,endometrial thickness and follicular diameter between the two groups(P>0.05);after treatment,the resistance index and pulsatility index of the two groups were lower than those before treatment,the endometrial thickness and follicular diameter were larger than those before treatment,the differences were statistically significant(P<0.05);after treatment,the resistance index and pulsation index of the observation group were lower than those of the control group,the endometrial thickness and follicular diameter were greater than those of the control group,the differences were statistically significant(P<0.05).After 2 years of follow-up,the clinical pregnancy rate of the observation group was higher than that of the control group,and the recurrence rate was lower than that of the control group,the differences were statistically significant(P<0.05).Conclusion Laparoscopic surgery+GnRHa+Guizhi Fuling capsule in the treatment of infertile patients with ovarian chocolate cyst has a remarkable effect,it can improve the clinical pregnancy rate,reduce recurrence rate,which is worthy of clinical promotion and application.
