摘要
目的:对比研究促性腺激素释放激素激动剂(GnRH-a)与地诺孕素治疗子宫内膜异位症的临床效果及疼痛。方法:回顾性选取航天中心医院2019年1月-2021年1月收治的子宫内膜异位症患者480例,根据治疗方式不同分成两组,GnRH-a组(n=290)皮下注射醋酸亮丙瑞林微球治疗,地诺孕素组(n=190)口服地诺孕素片治疗,比较两组临床疗效、疼痛情况、性激素及抗原水平。结果:GnRH-a组总有效率为78.62%,低于地诺孕素组的90.00%,差异有统计学意义(P<0.05)。地诺孕素组治疗后性交疼痛、痛经、非经期时下腹疼痛评分及孕酮(P)、雌二醇(E)、糖类抗原125(CA125)、糖类抗原199(CA199)水平均低于GnRH-a组,差异均有统计学意义(P<0.05)。GnRH-a组不良反应发生率为17.93%,地诺孕素组为4.74%,GnRH-a组不良反应发生率高于地诺孕素组,差异有统计学意义(P<0.05)。结论:地诺孕素比GnRH-a治疗子宫内膜异位症的临床效果更佳,能有效改善患者疼痛,且降低性激素及CA125、CA199水平,安全性也更高。
Objective:To compare the clinical effect and pain of Gonadotropin-releasing Hormone Agonist(GnRH-a)and Dienogest in the treatment of endometriosis.Method:A total of 480 patients with endometriosis admitted to Aerospace Center Hospital from January 2019 to January 2021 were retrospectively selected and divided into two groups according to different treatment methods.GnRH-a group(n=290)was treated with subcutaneous injection of Leprorelin Acetate Microspheres,and Dienogest group(n=190)was treated with oral Dienogest Tablets.The clinical efficacy,pain,sex hormone and antigen levels were compared between two groups.Result:The total effective rate of GnRH-a group was 78.62%,which was lower than 90.00%of Dienogest group,the difference was statistically significant(P<0.05).The scores of pain during intercourse,dysmenorrhea and non-menstrual lower abdominal pain,levels of progesterone(P),estradiol(E),carbohydrate antigen 125(CA125)and carbohydrate antigen 199(CA199)of GnRH-a group after treatment were lower than those of than Dienogest group,the differences were statistically significant(P<0.05).The incidence of adverse reactions in GnRH-a group was 17.93%,and Dienogest group was 4.74%,the incidence of adverse reactions in GnRH-a group was higher than that in Dienogest group,and the difference was statistically significant(P<0.05).Conclusion:Dienogest has better clinical effect than GNRH-a in the treatment of endometriosis,effectively improve the pain of patients,and reduce the levels of sex hormones and CA125,CA199,with higher safety.
