摘要
目的研究手术、化疗和放疗对胃癌患者营养状态的影响及其与贫血发生、发展的关系。方法选取448例手术治疗的胃癌患者(手术组),223例接受化疗的胃癌患者(化疗组),50例接受放疗的胃癌患者(放疗组)作为研究对象。测定三组治疗前后的红细胞计数(RBC)、血红蛋白(Hb)、白蛋白(ALB)和前白蛋白水平,比较三组患者治疗前后RBC、Hb、ALB和前白蛋白水平以及贫血发生情况,并分析三组患者ALB、前白蛋白水平与RBC、Hb水平的相关性。结果三组治疗后的RBC、Hb、ALB和前白蛋白水平均明显低于本组治疗前,差异均有统计学意义(P<0.05)。手术组、化疗组、放疗组治疗前贫血发生率分别为42.0%、49.8%、24.0%,治疗后分别为65.0%、78.9%、66.0%;三组治疗后的贫血发生率均明显高于本组治疗前,差异均有统计学意义(P<0.05)。三组患者的ALB、前白蛋白水平均与RBC和Hb水平呈正相关(P<0.05)。结论手术、化疗和放疗均会引起ALB和前白蛋白水平降低,同时引起RBC和Hb水平降低、贫血发生率升高;RBC和Hb水平均与ALB和前白蛋白水平呈负相关;手术、化疗和放疗均是引起胃癌患者贫血的重要原因。
Objective To study the effects of surgery,chemotherapy and radiotherapy on nutritional status of gastric cancer patients and their correlation with the occurrence and development of anemia.Methods 448 patients with gastric cancer treated with surgery(surgery group),223 patients with gastric cancer treated with chemotherapy(chemotherapy group),and 50 patients with gastric cancer treated with radiotherapy(radiotherapy group)were selected for the study.The red blood cell count(RBC),hemoglobin(Hb),albumin(ALB)and prealbumin levels were measured before and after treatment in the three groups,and the levels of RBC,Hb,ALB and prealbumin before and after treatment and the occurrence of anemia were compared in the three groups.The correlation of ALB and prealbumin levels with RBC and Hb levels in three groups was analyzed.Results The levels of RBC,Hb,ALB and prealbumin in the three groups after treatment were significantly lower than those before treatment in this group,and the differences were statistically significant(P<0.05).The incidences of anemia in the surgery group,chemotherapy group and radiotherapy group were 42.0%,49.8%,and 24.0%before treatment,and 65.0%,78.9%,and 66.0%after treatment,respectively;the incidence of anemia after treatment in the three groups was significantly higher than that before treatment in this group,and the differences were statistically significant(P<0.05).The levels of ALB and prealbumin in the three groups were positively correlated with the levels of RBC and Hb(P<0.05).Conclusion Surgery,chemotherapy and radiotherapy all cause a decrease in ALB and pre-albumin levels,along with a decrease in RBC and Hb levels and an increase in the incidence of anaemia;both RBC and Hb levels are negatively correlated with ALB and prealbumin levels;surgery,chemotherapy and radiotherapy are all important causes of anemia in patients with gastric cancer.
