摘要
目的 研究2型糖尿病合并广泛耐药鲍氏不动杆菌肺炎患者接受胰岛素泵与替加环素联合治疗的临床效果。方法 选取2020年1月—2021年10月包头医学院第一附属医院收治的70例2型糖尿病合并广泛耐药鲍氏不动杆菌肺炎患者,使用随机抽签法均等划分为两组,每组35例。观察组使用胰岛素泵联合替加环素治疗,对照组使用胰岛素皮下注射联合替加环素治疗。对比两组疗效。结果 观察组治疗后HOMA-β(76.14±9.24)高于对照组,HOMA-IR(1.89±0.51)低于对照组,差异有统计学意义(t=2.720、3.016,P<0.05);观察组治疗后空腹血糖(5.82±1.03)mmol/L、餐后2 h血糖(7.12±0.77)mmol/L、睡前血糖(6.14±1.01)mmol/L、糖化血红蛋白(6.11±0.54)%均低于对照组,差异有统计学意义(t=2.530、2.631、2.899、3.035,P<0.05);观察组临床总疗效97.14%高于对照组,差异有统计学意义(χ~2=5.080,P<0.05)。结论 使用胰岛素泵联合替加环素对2型糖尿病合并广泛耐药鲍氏不动杆菌肺炎患者实施治疗,可改善其β细胞功能,降低血糖指标,提升临床疗效。
Objective To study the clinical effect of insulin pump combined with tigecycline in patients with type 2diabetes mellitus complicated with extensively drug-resistant Acinetobacter baumannii pneumonia.Methods A total of 70 patients with type 2 diabetes and extensively drug-resistant Acinetobacter baumannii pneumonia admitted to the First Affiliated Hospital of Baotou Medical College from January 2020 to October 2021 were selected.They were equally divided into two groups by random drawing,with 35 cases in each group.The observation group was treated with insulin pump combined with tigecycline;the control group was treated with insulin subcutaneous injection combined with tigecycline.The therapeutic effects of the two groups were compared.Results After treatment,the HOMA-β(76.14±9.24) of the observation group was higher than that of the control group,and the HOMA-IR(1.89±0.51) was lower than that of the control group,and the differences were statistically significant(t=2.720,3.016,P<0.05).After treatment,fasting blood glucose(5.82±1.03) mmol/L,2 h postprandial blood glucose(7.12±0.77) mmol/L,bedtime blood glucose(6.14±1.01) mmol/L,glycosylated hemoglobin(6.11±0.54)% in observation group were lower than the control group,and the difference was statistically significant(t=2.530,2.631,2.899,3.035,P<0.05).The total clinical efficacy of the observation group was 97.14% higher than that of the control group,and the difference was statistically significant(χ~2=5.080,P<0.05).Conclusion The use of insulin pump combined with tigecycline in the treatment of patients with type 2 diabetes mellitus complicated with extensively drug-resistant Acinetobacter baumannii pneumonia can improve β-cell function,reduce blood glucose indicators,and improve clinical efficacy.
