摘要
目的探讨黄连解毒汤加减联合耳穴埋针治疗2型糖尿病(T2DM)胰岛素抵抗患者的效果。方法选取河南中医药大学第一附属医院T2DM胰岛素抵抗患者60例作为研究对象,采用随机数字表法分为对照组(n=30)和研究组(n=30)。两组均行饮食、运动干预,对照组采用二甲双胍治疗,研究组于对照组基础上采用黄连解毒汤加减联合耳穴埋针治疗,两组均持续治疗3个月。对比两组治疗效果、不良反应发生情况、治疗前后中医证候积分、胰岛功能指标[胰岛β细胞功能指数(HOMA-β)、胰岛素抵抗指数(HOMA-IR)]变化情况。结果研究组治疗总有效率[96.67%(29/30)]与对照组[80.00%(24/30)]比较,差异无统计学意义(P>0.05);治疗结束后,两组多食易饥、日渴喜饮、手足心热、小便频多积分均低于治疗前,且研究组低于对照组,差异有统计学意义(P<0.05);两组HOMA-β、HOMA-IR均优于治疗前,且研究组HOMA-β高于对照组,HOMA-IR低于对照组,差异有统计学意义(P<0.05)。两组均无明显不良反应发生。结论黄连解毒汤加减联合耳穴埋针辅助治疗T2DM胰岛素抵抗患者,能进一步提高患者胰岛功能,改善患者临床症状,安全性高。
Objective To investigate the effect of Huanglian Jiedu decoction combined with auricular acupoint embedding acupuncture on insulin resistance in patients with type 2 diabetes mellitus(T2DM).Methods Sixty patients with insulin resistance of T2DM in the First Affiliated Hospital of Henan University of Traditional Chinese Medicine were selected and divided into control group(n=30)and study group(n=30)by random number table method.Both groups were given diet and exercise intervention.The control group was treated with metformin,and the study group was treated with Huanglian Jiedu decoction combined with auricular acupoint embedding acupuncture on the basis of the control group.Both groups were treated for 3 months.The treatment effects,occurrence of adverse reactions,TCM syndrome scores,changes in islet function indexes[isletβcell function index(HOMA-β),insulin resistance index(HOMA-IR)]before and after treatment were compared between the two groups.Results The total effective rate of treatment in the study group was[96.67%(29/30)]and in the control group was[80.00%(24/30)],with no statistically significant difference between the two groups(P>0.05).After treatment,the scores of eating more and hungry,thirsting more and drinking more,fever of hands,feet and heart,frequent urination in both groups were lower than those before treatment,and those in the study group were lower than those in the control group(P<0.05).Both HOMA-βand HOMA-IR were better than before treatment,and HOMA-βin the study group was higher than that in the control group,and HOMA-IR was lower than the control group(P<0.05).No obvious adverse reactions occurred in the two groups.Conclusion Huanglian Jiedu decoction combined with auricular acupoint embedding acupuncture in the treatment of patients with T2DM insulin resistance can further improve the function of the pancreatic islets and improve the clinical symptoms of the patients with high safety.
