摘要
【目的】运用现代数据挖掘技术分析针刺治疗老年抑郁症的选穴规律及特点,为临床治疗老年抑郁症提供参考。【方法】采用计算机检索中国期刊全文数据库(CNKI)、万方学术期刊全文数据库(Wanfang)、维普中文科技期刊数据库(VIP)、中国生物医学文献数据库(SinoMed)、美国医学在线(PubMed)、荷兰医学文献数据库(Embase)及核心合集数据库(Web of Science)等数据库,从建库至2021年11月2日收录的针刺治疗老年抑郁症的相关文献,采用Microsoft Excel 2013软件建立老年抑郁症针刺处方数据库并进行描述性分析,运用IBM SPSS Modeler 18.0软件及IBM SPSS Statistics 24.0软件进行关联规则分析和系统聚类分析。【结果】共纳入41篇文献,52组针刺处方,涉及34个腧穴,总频次为344次。针刺治疗老年抑郁症最常使用的穴位分别是百会、印堂、内关、太冲、三阴交,最常取用督脉、足太阳膀胱经、手厥阴心包经的腧穴,临床针刺治疗部位多选用头部、四肢、背部。由关联规则分析表明,针刺治疗老年抑郁症的核心腧穴组为百会-印堂-三阴交-内关,最后对高频穴位进行聚类分类获得两个聚类群,分别是“百会、印堂、内关、三阴交、太冲、合谷、神门、风池、足三里、四神聪、太阳”和“肝俞、肾俞、心俞、脾俞”。【结论】针刺老年抑郁症取穴多选取督脉、足太阳膀胱经,以头部以及背部穴位为主,建议以百会-印堂-三阴交-内关作为核心穴位,配合辨证取穴。
Objective To analyze the acupoint selection rules and characteristics of acupuncture in the treatment of late-life depression(LLD)based on modern data-mining technology,so as to provide reference for clinical treatment of LLD.Methods Computer searches of the China National Knowledge Internet(CNKI),Wanfang Data Knowledge Service Platform(Wanfang),China Science and Technology Journal Database(VIP),SinoMed,PubMed,The Exerpta Medical Database(Embase)and Web of Science and other databases,and the related literatures of acupuncture treatment of LLD were screened from the establishment of the database to November 2,2021.Microsoft Excel 2013 software was used to build a database of LLD needling prescriptions and to conduct descriptive analysis,and IBM SPSS Modeler 18.0 software and IBM SPSS Statistics 24 software were used to conduct association rule analysis and systematic clustering analysis.Results A total of 41 articles with 52 sets of acupuncture prescriptions involving 34 acupoints and a total frequency of 344 were included in this study.The most frequently used acupoints for the treatment of LLD were Baihui(DU20),Yintang(EX-HN3),Neiguan(PC6),Taichong(LR3)and Sanyinjiao(SP6),acupoints from the governor vessel(GV),foot taiyang bladder meridians and the pericardium meridians of hand-jueyin were most frequently selected,and clinical acupuncture treatment sites were mostly chosen for the head,limbs and back.The analysis of the association rules showed that the core acupoints combination of acupuncture for LLD were Baihui(DU20)-Yintang(EX-HN3)-Sanyinjiao(SP6)-Neiguan(PC6),and finally two clusters were obtained by clustering the high frequency acupoints,which were“Baihui(DU20),Yintang(EX-HN3),Neiguan(PC6),Sanyinjiao(SP6),Taichong(LR3),Hegu(LI4),Shenmen(HT7),Fengchi(GB20),Zusanli(ST36),Sishencong(EX-HN1)and Taiyang(EX-HN5)”,and“Ganshu(BL18),Shenshu(BL23),Xinshu(BL15),and Pishu(BL20)”,respectively.Conclusion Acupoint selections for LLD are mostly selected from the GV and foot taiyang bladder meridians with the head and back points as the main points,and Baihui(DU20)-Yintang(EX-HN3)-Sanyinjiao(SP6)-Neiguan(PC6)are suggested as the core acupoints to coordinate with syndrome differentiation.
