摘要
目的:探讨玉屏风颗粒联合左西替利嗪治疗脾虚湿蕴型特应性皮炎(AD)的临床效果及对患儿湿疹面积及严重度指数(EASI)评分、复发率的影响。方法:选取2019年8月-2021年8月江西省儿童医院收治的78例脾虚湿蕴型AD患儿,按随机数字表法分为两组,各39例。对照组予以左西替利嗪治疗,观察组加用玉屏风颗粒治疗,持续用药4周。比较两组临床疗效、EASI评分、特应性皮炎SCORAD评分、血清学指标[白介素-4(IL-4)、免疫球蛋白E(IgE)]、复发率及不良反应发生情况。结果:治疗后,观察组总有效率高于对照组,EASI评分、SCORAD评分均低于对照组,差异均有统计学意义(P<0.05)。治疗后,观察组IL-4、IgE水平均低于对照组,差异均有统计学意义(P<0.05)。观察组复发率为5.13%,低于对照组的23.08%,差异有统计学意义(P<0.05)。结论:玉屏风颗粒联合左西替利嗪可提高脾虚湿蕴型AD患儿治疗效果,降低EASI评分、SCORAD评分,加快血清IL-4、IgE水平恢复正常,减少疾病复发,安全可靠。
Objective:To investigate the clinical effect of Yupingfeng Granules combined with Levocetirizine in the treatment of atopic dermatitis (AD) with spleen deficiency and dampness accumulation,and its effect on eczema area and severity index (EASI) score and recurrence rate.Method:A total of 78 children with AD of spleen deficiency and dampness accumulation type treated in Jiangxi Provincial Children’s Hospital from August 2019 to August 2021 were selected,they were randomly divided into two groups according to random number table,with 39 cases in each group.The control group was treated with Levocetirizine,and the observation group was treated with Yupingfeng Granules for 4 weeks.The clinical efficacy,EASI scores,scoring atopic dermatitis (SCORAD) scores,serological indexes[interleukin-4 (IL-4),immunoglobulin E (IgE)],recurrence rate and adverse reactions were compared between the two groups.Result:After treatment,the total effective rate of the observation group was higher than that of the control group,and the EASI scores and SCORAD scores of the observation group were lower than those of the control group,the differences were statistically significant (P<0.05).After treatment,the levels of IL-4 and IgE of the observation group were lower than those of the control group,the differences were statistically significant (P<0.05).The recurrence rate of the observation group was 5.13%,which was lower than 23.08% of the control group,the difference was statistically significant (P<0.05).Conclusion:Yupingfeng Granules combined with Levocetirizine can improve the therapeutic effect of AD children with spleen deficiency and dampness accumulation,reduce EASI scores and SCORAD scores,accelerate the normalization of serum IL-4 and IgE levels and reduce disease recurrence,it is safe and reliable.
