摘要
目的 探讨血清膜联蛋白A1(AnxA1)、γ-谷氨酰转肽酶(GGT)联合CT冠状动脉成像(CTA)在无典型症状冠心病诊断中的价值。方法 选取2020年1月至2021年6月在郑州市第七人民医院就诊的无典型症状冠心病患者92例,行CTA及冠状动脉造影(CAG)检查,同时检测血清AnxA1、GGT水平。结果 经CAG检查确诊,轻度狭窄患者62例,中度狭窄患者30例。CTA诊断患者冠状动脉狭窄程度与CAG结果 Kappa值为0.491,P<0.05。中度狭窄患者血清AnxA1、GGT水平分别为(2.22±0.64)μg/L和(45.50±9.92)U/L,明显高于轻度狭窄患者(P<0.05)。血清AnxA1、GGT水平诊断中度狭窄程度的AUC值分别为0.684(95%CI:0.617-0.738)、0.875(95%CI:0.726-0.931),P<0.05。血清AnxA1、GGT水平联合CTA诊断冠状动脉狭窄程度与CAG结果 Kappa值为0.765,联合诊断冠状动脉狭窄程度AUC值分别为0.897(95%CI:0.816-0.943),P<0.05。结论 血清AnxA1、GGT与无典型症状冠心病患者冠状动脉狭窄程度有一定关系,联合CTA诊断冠状动脉狭窄程度有较好的价值。
Objective To investigate the value of serum AnxA1,GGT combined with CTA in the diagnosis of asymptomatic coronary heart disease.Methods 92patients with asymptomatic coronary heart disease treated in our hospital from January 2020to June 2021were examined by CTA and CAG,and the levels of serum AnxA1and GGT were detected at the same time.Results The diagnosis was confirmed by CAG examination.There were 62patients with mild stenosis and 30patients with moderate stenosis.The degree of coronary artery stenosis diagnosed by CTA and the CAG result Kappa value was 0.491,P<0.05.Serum AnxA1and GGT levels in patients with moderate stenosis were(2.22±0.64)μg/L and(45.50±9.92)U/L,respectively,significantly higher than those in patients with mild stenosis(P<0.05).The AUC values of serum AnxA1and GGT levels in the diagnosis of moderate stenosis were 0.684(95%CI:0.617-0.738)and 0.875(95%CI:0.726-0.931),respectively(P<0.05).The Kappa value of serum AnxA1and GGT levels combined with CTA in the diagnosis of coronary stenosis and CAG results was 0.765,and the AUC value of the combined diagnosis of coronary stenosis was 0.897(95%CI:0.816-0.943),respectively(P<0.05).Conclusion There is a certain relationship between serum AnxA1,GGT and the degree of coronary artery stenosis in patients with asymptomatic coronary heart disease,combined with CTA,it is of great value in diagnosing the degree of coronary artery stenosis.
