摘要
目的探讨联合检测同型半胱氨酸(homocysteine,Hcy)与血管活性物质12(Apelin-12)在冠心病诊断中的协同作用,为临床提供更优化的无创筛查方案。方法选取2022年7月至2023年6月医院经冠脉造影确诊的冠心病患者52例(观察组)及健康体检者52例(对照组),采集两组受试者静脉血,检测两组血清Hcy(速率法)及Apelin-12(ELISA法)水平,分析Hcy、Apelin-12单独或联合检测的诊断效能(灵敏度、特异性、准确度及ROC曲线下面积),并分析Hcy和Apelin-12的相关性。结果观察组Hcy水平显著高于对照组(18.45±1.57μmol/L vs 11.63±1.24μmol/L,P<0.001),观察组Apelin-12水平显著低于对照组(0.94±0.15μg/L vs 2.31±0.26μg/L,P<0.001),Hcy和Apelin-12呈负相关(r=-0.732,P<0.001)。Hcy与Apelin-12联合检测的灵敏度(94.23%)、特异性(92.31%)及准确度(93.27%)均显著优于单一检测(P<0.05),联合检测ROC曲线下面积(AUC=0.96)高于Hcy(AUC=0.82)和Apelin-12(AUC=0.84)。结论Hcy与Apelin-12呈负相关,联合检测在冠心病人群中具有较高的检测效能,这或许能作为筛查冠心病患者的潜在生物学标志物的补充。
Objective To evaluate the synergistic effect of combined detection of Homocysteine(Hcy)and vasoactive substance 12(Apelin-12)in the diagnosis of coronary heart disease,so as to provide an optimized non-invasive screening program for clinical practice.Methods From July,2022 to June,2023,52 patients with coronary heart disease confirmed by coronary angiography(observation group)and 52 healthy subjects(control group)were selected for the study.Venous blood samples of all subjects from both groups were collected,and serum Hcy(rate method)and Apelin-12(ELISA method)levels were detected.The diagnostic efficacy(sensitivity,specificity,accuracy and area under ROC curve)of Hcy and Apelin-12 alone or in combination were analyzed,and the correlation between Hcy and Apelin-12 was evaluated.Results Hcy level in the observation group was significantly higher than that in the control group(18.45±1.57 vs 11.63±1.24μmol/L,P<0.001),while the Apelin-12 level in the observation group was significantly lower than that in the control group(0.94±0.15 vs 2.31±0.26μg/L,P<0.001).Hcy was negatively correlated with Apelin-12(r=-0.732,P<0.001).The sensitivity(94.23%),specificity(92.31%)and accuracy(93.27%)of combined detection of Hcy and Apelin-12 were significantly better than those of single-marker detection(P<0.05).The area under the combined ROC curve(AUC=0.96)was higher than that of Hcy(AUC=0.82)or Apelin-12(AUC=0.84)alone.Conclusion There is a negative correlation between Hcy and Apelin-12,and the 2-marker combined test shows a high detection efficiency in the coronary heart disease population,which could serve as a potential biomarker for screening patients with coronary heart disease.
