摘要
目的 针对股骨近端防旋髓内钉治疗老年股骨转子间骨折的疗效进行分析研究。方法 截取大理市第一人民医院在2016年1月-2020年1月收治的140例老年股骨转子间骨折患者作为研究对象,采用随机分组的原则,将选取的患者分为2组,每组70例。其中,采取解剖型锁定钢板(PFLAP)治疗的患者设置为对照组,采取股骨近端防旋髓内钉(PFNA)治疗的患者设置为干预组。对两组患者围手术期的相关指标进行观察,其包含了术中出血量、切口长度、手术时间、术后负重时间、骨折愈合时间。同时,记录病患手术后并发症的发生情况以及髋关节功能的情况。结果 干预组病患术中出血量、切口长度、手术时间、术后负重时间等指标相较于对照组,差异显著(P<0.05);干预组患者的骨折愈合时间相较于对照组,差异无统计学意义(P>0.05)。对照组病患术后并发症发生率高于干预组,其差异没有统计学意义(P<0.05)。结论 对老年股骨转子间骨折患者,采取股骨近端防旋髓内钉治疗的临床效果显著,且围术期各项指标有所优化,对患者创伤小,值得在临床上进行推广。
Objective To analyze curative effect of proximal femoral anti rotation intramedullary nail in treatment of elderly femoral intertrochanteric fractures.Methods The paper chose 140 cases elderly patients with femoral intertrochanteric fracture treated in our hospital from January 2016 to January 2020 as research objects,and divided them into two groups with principle of random grouping,with 70 cases in each group.Among them,patients in control group were treated with anatomical locking plate (PFLAP),and patients in intervention group with proximal femoral anti rotation intramedullary nail (PFNA).Perioperative related indexes between two groups were observed,including intraoperative bleeding,incision length,operation time,postoperative weight-bearing time and fracture healing time.At the same time,incidence of postoperative complications and hip function were recorded.Results There was significant difference in intraoperative bleeding,incision length,operation time and postoperative weight-bearing time between intervention group and control group,(P<0.05);there was no significant difference in fracture healing time between intervention group and control group (P>0.05).Incidence of postoperative complications in control group was higher than intervention group,difference was not statistically significant (P<0.05).Conclusion Proximal femoral anti rotation intramedullary nail can achieve significant clinical effect in treatment of elderly patients with femoral intertrochanteric fracture,can optimize perioperative indexes,with little trauma,which is worthy of clinical promotion.
