摘要 目的对比观察防旋股骨髓内钉(PFNA)与动力髋螺钉(DHS)治疗老年股骨粗隆间骨折疗效,探讨内固定选择在老年股骨粗隆间骨折治疗中的意义。方法回顾总结本院自2008年6月~2011年2月分别采用PFNA及DHS内固定方法治疗108例老年股骨粗隆间骨折患者;使用PFNA治疗46例、DHS治疗62例,从手术时间、术中出血量、术后负重时间、骨折愈合时间、术后并发症发生率、髋关节活动度、行走情况进行对比,分析比较两种内固定方法的优缺点。结果 108例患者均得到随访,随访时间9~18个月,两组在平均手术时间、术中出血、骨折愈合时间、术后并发症发生率及髋关节功能方面有统计学差异(P<0.05);PFNA组优于DHS组。结论 PFNA较DHS设计更合理、操作微创,固定牢固,是治疗老年股骨粗隆间的理想方法之一。 Objective To compare the effect of proximal femoral nail antirotation (PFNA) and dynamic hip screw I DHS) in treatment of intertrochanteric fractures so as to explore the significance of internal fixation in the treatment of elderly intertroehanterie fracture. Methods Data of 118 intertroehanterie fracture patients admitted into our hospital from Jun. 2008 to Feb. 2011 were retrospectively analyzed,who were treated with PFNA and DHS. Among the 118 patients ,46 patients were treated by PFNA therapy,62 patients by DHS. The operation time, blood loss, postoperative weight, fracture healing time, incidence of postoperative complications, hip activity and walking situation were compared, so as to analyze the advantages and disadvantages of the two internal fixation methods. Results Totally 108 patients were followed up for 9-18 months. There was statistical difference in average operative time, blood loss, fracture healing time, incidence of postoperative complications and hip function between the two groups ( P 〈 O. 05 ) ; the curative effect in the PFNA group is better than that of the DHS group. Conclusion PF- NA way for treating elderly intertrochanteric fracture patients has advantages of reasonable design, mini-invasiveness, and reliable fixation. It is an ideal operation method for treating intertroehanteric fractures in elderly patients.
