普罗帕酮与胺碘酮治疗对冠心病心律失常患者心电图和血清NT-proBNP的影响 被引量:5

Effects of Propafenone and Amiodarone on Electrocardiogram and Serum NT-proBNP in Patients with Coronary Heart Disease and Arrhythmia

摘要 目的:探讨普罗帕酮与胺碘酮治疗对冠心病心律失常患者心电图和血清N末端脑钠肽前体(NT-proBNP)的影响。方法:选取2020年1月-2020年12月在某院确诊的76例冠心病心律失常患者,采用数字简单随机分组的方法将患者分为观察组和对照组各38例,对照组予以普罗帕酮治疗,观察组予以胺碘酮治疗,均持续治疗8周。评估两组患者的临床疗效;分别于治疗前后,采用动态心电图方法来检测房性期前收缩(PAC),室性期前收缩(VPC)以及心房颤动(AF)发作次数;采用酶联免疫吸附试验法检测患者肌酸激酶(CK)水平,采用化学发光法检测患者NT-proBNP及脑钠肽(BNP)水平;记录患者不良反应发生率。结果:观察组总有效率为92.11%,高于对照组的73.68%(P<0.05);治疗后,观察组患者的PAC、VPC以及AF发作次数明显低于对照组(P<0.05);观察组CK、NT-proBNP以及BNP明显低于对照组(P<0.05);观察组不良反应发生率为15.79%,与对照组的26.32%比较差异无统计学意义(P>0.05)。结论:相较于普罗帕酮,胺碘酮用于冠心病心律失常患者的疗效更好,可有效缓解患者心律失常症状,改善心电图的诊断检测结果,降低NT-proBNP水平。 Objective:To explore the effects of propafenone and amiodarone on electrocardiogram and serum N-terminal pro-brain natriuretic peptide(NT-proBNP)in patients with coronary heart disease and arrhythmia.Methods:A total of 76 patients confirmed with coronary heart disease and arrhythmia in a hospital from January to December 2020 were selected and divided into observation group(n=38)and control group(n=38)by simple numerical random grouping method.The control group was treated with propafenone,while the observation group was treated with amiodarone.The two groups were continuously treated for 8 weeks.The clinical curative effect in both groups was evaluated.Before and after treatment,attack times of premature atrial contraction(PAC),ventricular premature contraction(VPC)and atrial fibrillation(AF)were detected by dynamic electrocardiogram.The level of creatine kinase(CK)was detected by enzyme-linked immunosorbent assay.The levels of NT-proBNP and brain natriuretic peptide(BNP)were detected by chemiluminescence method.The incidence of adverse reactions was recorded.Results:The total effective rate of treatment in observation group was significantly higher than that in control group(92.11%vs 73.68%)(P<0.05).After treatment,attack times of PAC,VPC and AF in observation group were significantly lower than those in control group(P<0.05).CK,NT-proBNP and BNP in observation group were significantly lower than those in control group(P<0.05).The difference in the incidence of adverse reactions between observation group and control group was not statistically significant(15.79%vs 26.32%)(P>0.05).Conclusion:Compared with propafenone,curative effect of amiodarone is better in patients with coronary heart disease and arrhythmia,which can effectively relieve arrhythmia symptoms,improve test results of electrocardiogram and reduce NT-proBNP level.
