摘要
目的:探讨卢益萍教授运用抗敏方治疗风寒型荨麻疹的经验。方法:通过门诊跟师侍诊,记录和整理卢益萍教授运用抗敏方治疗风寒型荨麻疹的医案,并结合古籍及现代文献研究,从抗敏方所主风寒型荨麻疹的病因病机及其方义探微分析总结卢益萍教授辨证治疗风寒型荨麻疹的临床经验,并附验案一则。结果:卢益萍教授认为,风寒型荨麻疹的病因病机和临床症状多与气虚相关,并认为禀赋不耐、脾胃失养、营卫失和是风寒型荨麻疹发病之本,复感风寒邪气为致病之标,主张以解表散寒,益气固表为主要治法,兼顾补脾和胃,活血祛风。而表虚不固、营卫不和,风寒束表也是抗敏方证的病机关键,与风寒型荨麻疹病机符合,因此卢教授临证选用抗敏方加减治疗风寒型荨麻疹。所附医案,卢教授辨为素体气虚,风寒束表,治疗用抗敏方加减,取得较好效果。结论:卢益萍教授治疗风寒型荨麻疹,紧扣“表虚不固、营卫失和、风寒束表”的病机,运用抗敏方加减以解表邪,扶正气,和营卫,可有效改善症状,减少复发,为临床治疗提供了新选择。
Objective:To explore clinical experience of Professor LU Yiping in treating the wind-cold type urticaria with the Kangmin prescription(抗敏方).Methods:As the master’s assistant,the author learned medical skills from the master,Professor LU Yiping,and recorded and collated the medical records of treating the wind-cold type urticaria with the Kangmin prescription by Professor LU Yiping.Based on the studies on ancient books and modern literature,from the aspects of the etiological factor and pathogenesis of the wind-cold type(风寒型)ruticaria and the significance of the Kangmin prescription,the clinical experience of Professor LU Yiping in treating the wind-cold type urticaria was analyzed and summarized.And an empirical case is attached.Results:Professor LU Yiping thought that the etiology and pathogenesis and clinical symptoms of the wind-cold type urticaria is related to Qi(气)deficiency.Insufficient endowment,inadequate nourishment of the spleen and stomach,and health system maladjustment were considered as the root causes of the wind-cold type urticaria,and repeated exposure to wind and coldness was considered as the superficial cause of the disease.The method of removing superficial symptoms and dispelling coldness,replenishing Qi and solidifying body resistance was used as the main therapy,meanwhile invigorating the spleen and stomach,promoting blood circulation and eliminating wind were emphasized by Professor LU Yiping.And deficient body resistance,health system maladjustment,and exogenous symptoms due to wind-cold invasion were consistent with the pathogenesis of the wind-cold type urticaria,so Professor LU chose the Kangmin prescription to treat the wind-cold type urticaria.The case in the attached medical record was diagnosed as deficient body resistance and exogenous symptoms due to wind-cold invasion by Professor LU,the Kangmin prescription was applied to treat the disease,and the result was good.Conclusion:In the treatment of the wind-cold type urticaria,Professor LU Yiping closely followed the pathogenesis of“deficient body resistance,health system maladjustment,and exogenous symptoms due to windcold invasion”,and used the Kangming prescription to relieve surface symptoms,enhance body resistance,and strengthen vital energy,which can effectively ameliorate clinical symptoms and decrease recurrence.It provides a new choice for clinical treatment of the disease.
