摘要
目的探究肺泡气-动脉血氧分压差(A-aDO_(2))在肺尘埃沉着病患者中的诊断价值。方法选取2020年1月至12月韶关市职业病防治院收治的40例肺尘埃沉着病患者作为观察组,选取2020年1月至12月韶关市职业病防治院收治的具备相同粉尘接触条件的健康体检者40例设为对照组。检测两组的二氧化碳分压(PaCO_(2))、血氧分压(PaO_(2))、A-aDO_(2)、第1秒用力呼气容积(FEV1)和用力肺活量(FVC),并绘制ROC曲线,分析PaO_(2)、PaCO_(2)和A-aDO_(2)的诊断效能。结果观察组的PaO_(2)低于对照组,差异有统计学意义(P<0.05);观察组的PaCO_(2)和A-aDO_(2)高于对照组,差异有统计学意义(P<0.05)。观察组的FEV1、FVC低于对照组,差异有统计学意义(P<0.05)。肺尘埃沉着病患者PaO_(2)、PaCO_(2)、A-aDO_(2)的AUC和灵敏度分别为:0.788及86%、0.863及87%、0.873及98%。结论肺泡气-动脉血氧分压差在肺尘埃沉着病患者中具有较好的诊断价值,且A-aDO_(2)的AUC及灵敏度均高于PaCO_(2)和PaO_(2),早期监测A-aDO_(2)指标有助于早期确诊肺尘埃沉着病,评估尘肺患者的肺功能,判断疾病的进展和预后,从而及时治疗肺尘埃沉着病。
Objective To explore the diagnostic value of alveolar-arterial blood oxygen pressure(A-aDO_(2))difference in pneumoconiosis.Methods A total of 40 pneumoconiosis patients in Shaoguan occupational disease prevention and control hospital from January 2020 to December 2020 were selected as the observation group,and 40 patients with the same dust contact conditions admitted in Shaoguan Occupational Disease Prevention Hospital from January to December 2020 were selected as control group.The partial pressure of carbon dioxide(PaCO_(2)),arterial oxygen partial pressure(PaO_(2)),A-aPO_(2),forced expiratory volume in one second(FEV1),and forced vital capacity(FVC)values of the two groups were detected,the ROC curve was drawn to analyze the diagnostic efficacy of PaCO_(2),PaO_(2) and A-aDO_(2).Results PaO_(2) in the observation group was lower than that in the control group,with statistical significant difference(P<0.05).PaCO_(2) and A-aDO_(2) in the observation group were higher than those in the control group,with statistical significant differences(P<0.05).FEV1 and FVC in the observation group were lower than those in the control group(P<0.05).The PaO_(2),PaCO_(2),A-aDO_(2) of pneumoconiosis patients′AUC and sensitivity were 0.788 and 86%,0.863 and 87%,0.873 and 98%.Conclusion A-aDO_(2) has good diagnostic value in pneumoconiosis patients.The AUC and sensitivity of A-aDO_(2) are significantly higher than those of PaCO_(2) and PaO_(2).Early monitoring of A-aDO_(2) is helpful to early diagnose pneumoconiosis,evaluate the pulmonary function of pneumoconiosis patients,judge the progress and prognosis of the disease,so as to treat pneumoconiosis in time.
