摘要
目的:探讨升陷汤加减联合信必可(布地奈德/福莫特罗粉吸入剂)治疗职业性尘肺病合并慢性阻塞性肺疾病(COPD)辨证为肺气虚型的临床疗效。方法:将80例职业性尘肺病合并COPD稳定期患者作为研究对象,随机分为两组,每组40例。在常规治疗的基础上,观察组给予升陷汤加减联合信必可治疗,对照组仅给予信必可吸入治疗,疗程为12周。观察两组治疗前后肺功能指标FEV_(1)、FVC、FEV_(1)%Pred,CAT评分、中医症候积分评分。结果:两组在治疗12周后,肺功能指标FEV_(1)、FVC、FEV_(1)%Pred均较前升高,且观察组明显高于对照组(P<0.05);CAT评分及中医症候积分均较治疗前降低,且观察组积分明显低于对照组(P<0.05),改善气短乏力、自汗畏风效果显著(P=0.000)。结论:中西医结合治疗职业性尘肺病合并COPD稳定期可明显提高肺功能指标FEV_(1)、FVC、FEV_(1)%Pred,降低CAT评分及中医症候积分,明显改善生活质量,优于单纯西医治疗。
Objective:To investigate the clinical efficacy of add and subtract Shengxian Decoction combined with SYMBICORT(Budesonide and Formoterol Powder for Inhalation)in the treatment of patients with occupational pneumoconiosis combined with COPD(Lung Qi deficiency in Traditional Chinese Medicine syndrome differentiation).Methods:A total of 80 patients with occupational pneumoconiosis combined with COPD during the stable period were randomly divided into 2 groups,with 40 patients in each group.Under the condition of general routine treatment,the observation group was treated with add and subtract Shengxian Decoction combined with SYMBICORT,while the control group was treated only with SYMBICORT inhalation therapy.The course of treatment is 12 weeks.To observe the changes of lung function indexes FEV_(1),FVC,FEV_(1)%,Traditional Chinese Medicine syndrome score and CAT score before and after treatment.Results:After 12 weeks’treatment,the lung function indexes FEV_(1),FVC and FEV_(1)%Pred were all higher than before,and the observation group was significantly higher than the control group(P<0.05).Both CAT score and Traditional Chinese Medicine syndrome score were lower than before treatment,and the score of the observation group was significantly lower than that of the control group(P<0.05).Traditional Chinese Medicine syndrome score has a significant effect on improving symptoms such as shortness of breath,lassitude,spontaneous sweating and wind-fearing(P=0.000).Conclusion:Integrated traditional and western medicine can significantly improve the pulmonary function indexes FEV_(1),FVC and FEV_(1)%Pred,reduce the CAT score and Traditional Chinese Medicine syndrome score in the treatment of patients with occupational pneumoconiosis combined with COPD during the stable period,and significantly improve their quality of life.And it is superior to pure western medicine treatment.
