传动直丝弓技术非手术矫治骨性Ⅲ类患者的下切牙角度及牙槽骨形态学变化分析 被引量:14

Inclination change of lower incisiors and morphological changes of the alveolar bone in skeletal ClassⅢpatients treated by force-transmission technique

摘要 目的探讨传动直丝弓技术矫治骨性Ⅲ类患者的下切牙角度及其牙槽骨形态学变化.方法选择2013年1月至2021年1月在北京大学口腔医院正畸科进行传动直丝弓矫治的骨性Ⅲ类错患者12例,12例患者均有完整的治疗前后头颅侧位片及CBCT大视野数据.分析受试患者治疗前后下中切牙牙轴和颏轴,通过CBCT测量下中切牙周围的牙槽骨厚度(分别在距釉牙骨质界4、6、8mm处测量)、牙槽骨面积及垂直骨水平等,采用配对t检验进行统计学分析.结果治疗后下中切牙牙轴与颏轴一致性较好;治疗前下中切牙唇舌侧牙槽骨均较薄,唇侧各水平厚度平均值为1.51 mm,舌侧平均为0.71 mm,唇侧垂直骨水平平均为1.94 mm,舌侧为1.60 mm;治疗后唇侧、舌侧各水平牙槽骨厚度和高度均有所降低,但均保持有一定量的牙槽骨;此外,CBCT显示治疗后牙根长度仅有少量降低.结论本研究结果显示传动直丝弓矫治骨性Ⅲ类患者,其治疗后的下中切牙始终在基骨内,周围牙槽骨形态保持较好,同时无明显不健康的牙根吸收,提示可能与正畸过程中轻力产生的牙齿移动及牙槽骨良性改建有直接关系,也提示广大正畸医师健康矫治及持续轻力的重要性. Objective To investigate the changes of the angulation of lower incisors and alveolar bone morphology in skeletal class Ⅲ patients treated by force-transmission technique.Methods A total of 12 skeletal class Ⅲ patients were selected from the department o£orthodontics in Peking University Hospital of Stomatology from January 2013 to January 2021,and lateral cephalographs were taken before and after treatment.They had complete CBCT data before and after treatment.Inclination of lower central incisors were analyzed before and after treatment.The alveolar bone thickness(at levels of 4,6 and 8 mm from the enamel cementum junction),alveolar bone area and vertical bone level around the lower central incisor were measured by CBCT,and the paired t-test was used for statistical analysis.Results After treatment,the consistency between the long axis of the lower incisors and the long axis of the mandibular symphysis was maintained.Before treatment,the alveolar bone of the lower central incisors on both labial and lingual sides were thin.The mean thickness was 1.51 mm on labial side and 0.71 mm on lingual side.The mean vertical height of the alveolar bone was 1.94 mm on labial side and 1.60 mm on lingual side respectively.After treatment,both the thickness and height of alveolar bone at the labial and lingual sides were reduced,but a certain amount of alveolar bone was maintained.In addition,CBCT showed only a small reduction in root length after treatment,which was well-preserved after treatment.Conclusions This study showed that lower incisors of skeletal class U patients maintained within the base bone with no obvious root resorption after the treatment by force-transmission technique,and the surrounding alveolar bone still keep good morphology.It may be related to light-force-induced orthodontic tooth movement and physiological reconstruction pattern of the alveolar bone.It also indicates the importance of healthy treatment and light force during the orthodontic treatment.
