摘要
目的:观察揿针联合穴位贴敷治疗急性痛风性关节炎的临床疗效及对相关炎症因子表达的影响。方法:将74例急性痛风性关节炎患者随机法分为治疗组和对照组,每组37例。对照组给予美洛昔康片口服治疗,治疗组给予揿针联合穴位贴敷治疗。2组均以1周为1个疗程。观察2组临床疗效,以及治疗前后视觉模拟评分法(VAS)评分、C反应蛋白(CRP)、血尿酸(UA)、白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)水平。结果:治疗组显效18例,有效15例,无效4例,总有效率为89.19%;对照组显效12例,有效14例,无效11例,总有效率为70.27%。2组比较,差异有统计学意义(P<0.05)。治疗后,2组VAS评分及UA、CRP、IL-6、TNF-α水平较治疗前均降低(P<0.05);且治疗组低于对照组(P<0.05)。结论:揿针联合穴位贴敷治疗急性痛风性关节炎疗效确切,作用机制可能与调控UA、CRP水平,抑制血清炎症因子IL-6、TNF-α的表达有关。
Objective:To observe the clinical efficacy of pressing needle combined with acupoint application in the treatment of acute gouty arthritis and its effect on the expression of related inflammatory factors.Methods:Seventy-four patients with acute gouty arthritis were randomly divided into a treatment group and a control group,with 37 cases in each group.The control group was treated with meloxicam tablets orally,and the treatment group was treated with pressing needle combined with acupoint application.One week was a course of treatment for both groups.The clinical efficacy,visual analogue scale(VAS),C-reactive protein(CRP),uric acid(UA),interleukin-6(IL-6)and tumor necrosis factor(TNF-α)were observed before and after treatment.Results:In the treatment group,18 cases were markedly effective,15 cases were effective and 4 cases were ineffective,and the total effective rate was 89.19%;in the control group,12 cases were markedly effective,14 cases were effective and 11 cases were ineffective,and the total effective rate was 70.27%.There was significant difference between the two groups(P<0.05).After treatment,the levels of VAS,UA,CRP,IL-6 and TNF-αin the two groups were lower than those before treatment(P<0.05).Conclusion:Pressing needle combined with acupoint application is effective in the treatment of acute gouty arthritis,and its mechanism may be related to regulating the levels of UA and CRP and inhibiting the expressions of serum inflammatory factors IL-6 and TNF-α.
