摘要
目的探讨中青年痛风发病患者血糖、血压、血脂、肝肾功能及痛风相关指标与体质量指数(BMI)相关性,为痛风防治提供可借鉴依据。方法选取2015-01-01-2017-12-31北京市昌平区回龙观社区卫生服务中心收治的517例中青年痛风患者为研究对象。根据BMI将其分为正常体质量组(BMI<24kg/m2)及超重组(BMI≥24kg/m2)。采用Spearman分析超重组临床相关指标与BMI的相关性,并采用线性回归分析模型筛选BMI的影响因素。结果超重患者相关性分析结果显示,腰围(r=0.755,P<0.001)、收缩压(r=0.682,P<0.001)、舒张压(r=0.540,P=0.010)、空腹血糖(r=0.811,P<0.001)、TC(r=0.801,P<0.001)、TG(r=0.882,P<0.001)、谷丙转氨酶(r=0.663,P=0.001)、血尿酸(r=0.817,P<0.001)及肌酐清除率(r=0.851,P<0.001)均与BMI呈正相关。多因素Logistic回归分析结果显示,腰围(OR=2.776,P=0.020)、收缩压(OR=2.358,P<0.001)、空腹血糖(OR=3.117,P=0.031)、血尿酸(OR=2.264,P=0.001)及肌酐清除率(OR=3.459,P=0.001)均是BMI的影响因素。结论中青年痛风以超重患者居多,腰围、收缩压、空腹血糖、血尿酸及肌酐清除率等指标与BMI密切相关,重视相关因素的变化及影响,有助于中青年痛风患者的社区干预,改善患者预后。
Objective This study aimed to explore the correlation of blood glucose,blood pressure,blood lipids,liver and kidney function,gout-related indicators,etc and body mass index(BMI)in young and middle-aged gout patients,and provided a reference data for the prevention and treatment of gout.Methods Totally 517 young and middle-aged patients diagnosed with gout in Community Health Service Center of Huilongguan from January 1,2015 to December 31,2017 were included into this study.These patients divided into the normal weight group(BMI<24 kg/m2)and overweight group(BMI≥24 kg/m2)according to the BMI.The spearman analysis was used to examine the correlation between these clinical related indexes and BMI,and linear regression analysis was used to screen the influencing factors of BMI.Results The results of the correlation analysis showed that waist circumference(r=0.755,P<0.001),systolic blood pressure(r=0.682,P<0.001),diastolic blood pressure(r=0.540,P=0.010),and fasting blood glucose(r=0.811,P<0.001),TC(r=0.801,P<0.001),TG(r=0.882,P<0.001),ALT(r=0.663,P=0.001),blood uric acid(r=0.817,P<0.001),creatinine clearance rate(r=0.851,P<0.001)were significantly positively correlated with BMI in the overweight group.The results of regression analysis showed that waist circumference(OR=2.776,P=0.020),diastolic blood pressure(OR=2.358,P<0.001),fasting blood glucose(OR=3.117,P=0.031),blood uric acid(OR=2.264,P=0.001),creatinine clearance rate(OR=3.459,P=0.001)were independent influencing factors of BMI.Conclusions The majority of Young and middle-aged gout patients are overweight.The clinical indexes such as waist circumference,diastolic blood pressure,fasting blood glucose,uric acid and creatinine clearance rate are closely related to BMI.To pay more attention for changes and inflences of these indexes will be benefit for community intervention in young and middle-aged gout patients,and improve the prognosis.
