摘要
为了解广州地区患者在临床实践中共享决策的参与现状及相关影响因素,探讨推进医患共享决策的策略,采用问卷调查的形式对广州市6家医院的320名住院患者进行调查。通过对303份有效调查问卷进行统计学分析,结果发现55.78%的患者认为应该以合作形式进行医疗决策制定,且较多患者面对决策冲突时能够辩证看待问题。55.44%的患者属于积极决策者,44.56%的患者属于消极决策者。在患者参与医疗决策的影响因素方面,结果显示年龄越大的患者参与医疗决策的意愿越高,学历越高的患者越希望积极参与到医疗决策中。因此,推进医患共享决策,应考虑年龄、学历对患者参与决策意愿的影响,要注重鼓励患者积极参与决策、发挥“家庭参与”的积极作用、厘清决策权的配置和行使边界,以及强化医生共情能力和情感投入。
In order to understand the current situation and the related influencing factors of patients’participation in clinical shared decision-making practice in Guangzhou,and to explore the measures to promote the sharing decision-making between doctors and patients,a questionnaire survey was conducted among 320 inpatients in six hospitals in Guangzhou.Through statistical analysis of 303 valid questionnaires,the results showed that 55.78%of the patients believed that medical decision-making should be carried out in the form of cooperation,and more patients were able to treat problems dialectically when faced with decision-making conflicts.55.44%of the patients were positive decision makers,44.56%were negative decision makers.In terms of influencing factors of patients’participation in medical decision-making,the older the patients were,the more willing they were to participate in the medical decision-making,and the patients with higher educational background were more willing to participate in the medical decision-making.Therefore,it is suggested that the influence of age and education background on patients’willingness to participate in decision-making should be taken into account to promote doctor-patient sharing decision-making,and encouraged patients to actively participate in decision-making and play the positive role of“family participation”,the allocation and exercise boundaries of decision-making power should be clarified,as well as the empathetic ability and emotional input of doctors should be strengthened.
