摘要
目的:探讨对颈椎管狭窄症(CSS)采取不同的颈椎后路手术治疗的效果及对患者生活质量的影响。方法:选取60例CSS患者随机分为实验组与对照组,每组各30例,对照组采取经后路单开门椎管扩大成形术治疗,实验组采取经后路双开门椎管扩大成形术,比较两组手术治疗效果。结果:术后1个月两组患者颈总活动度(ROM)较术前降低,但实验组降低幅度与对照组比较差异无统计学意义(P>0.05),术后两组椎管管径较术前提高,且实验组高于对照组(P<0.05);术后6个月两组生理功能、心理功能、精神健康及社会功能各生活质量评分均较术前提高,且实验组高于对照组(P<0.05)。结论:针对CSS,采取经后路单开门、双开门椎管扩大成形的手术方法均可取得满意效果,但双开门的方式可更好提高患者生活质量。
Objective:To explore the effects of different posterior cervical spine surgery on cervical spinal stenosis(CSS)and the impact on the quality of life of patients.Methods:60 CSS patients were randomly divided into experimental group and control group,with 30 cases in each group.The control group was treated with posterior single-door open-door spinal canal enlargement and the experimental group was treated with double-opened open-door spinal canal expansion.The effect of surgical treatment in both groups were compared.Results:The total neck activity(ROM)of the two groups of patients decreased after operation 1 month,but the reduction in the experimental group was not statistically different from that in the control group(P>0.05).The spinal canal were expansed in both groups after operation.Preoperatively,the index of the experimental group was higher than that of the control group(P<0.05).The quality of life scores of the physiological function,psychological function,mental health and social function of the two groups were significantly higher after operation 6 months than before operation,the score of the experimental group was significantly higher than that of control group(P<0.05).Conclusion:For CSS,the operation method of posterior approach single-opening and double-opening vertebral canal expansion can achieve satisfactory results,but the double-opening method can better improve the quality of life of patients.
