摘要
目的探讨单纯椎板减压与后路椎间植骨融合内固定术治疗老年腰椎管狭窄症的效果及对病人生活质量的影响。方法选取2015年3月至2017年4月我院收治的老年腰椎管狭窄症病人50例,依据不同的治疗方式分为对照组和研究组,对照组采用单纯椎板减压治疗,研究组采用后路椎间植骨融合内固定术治疗,对比2组病人的治疗效果及病人的生活质量。结果研究组手术时间、术中失血量、住院时间明显低于对照组(P<0.05);治疗后研究组病人的优良率明显高于对照组(P<0.05);治疗前2组病人的疼痛评分差异无统计学意义(P>0.05),手术后均得到了一定的改善,研究组病人的疼痛评分明显低于对照组(P<0.05);治疗前2组病人的生活质量评分差异无统计学意义(P>0.05),手术治疗1年后生活质量评分都得到提高,研究组明显高于对照组(P<0.05)。结论对于老年腰椎管狭窄症病人,采用后路椎间植骨融合内固定术治疗的效果明显优于单纯椎板减压治疗,可以提高治疗优良率、降低疼痛感、提升病人的生活质量,值得推广和应用。
Objective To compare the clinical effect of decompressive laminectomy and posterior lumbar interbody fusion and to investigate the quality of life in the elderly patients with lumbar spinal stenosis. Methods Fifty cases of elderly patients with spinal tube stenosis from March 2015 to April2017 in our hospital were enrolled in this study,and they were divided into the study group and the control group according to the different treatment methods. The control group was treated with decompressive laminectomy treatment,and the study group was treated with posterior lumbar interbody fusion and internal fixation treatment. The clinical outcome and the quality of life were compared between two groups. Results The operation time,bleeding volume and hospitalization time in the study group were significantly lower than those in the control group（ P〈0. 05）. After treatment,the effective rate in the study group was significantly higher than that in the control group（ P〈0. 05）. Before treatment,the pain scores between two groups showed no significant difference（ P〉0. 05）; while after treatment,the pain scores in the study group were significantly lower than those in the control group（ P〈0. 05）. After treatment,the score of quality of life in the study group was significantly higher than that in the control group（ P〈0. 05）. Conclusions For elderly patients with lumbar spinal stenosis,the posterior lumbar interbody fusion has a better effect than decompressive laminectomy,which can efficiently improve the operation condition and the effective rate,reduce the pain degree and increase their quality of life.
