摘要
目的探讨两种颈椎后路手术方法治疗颈椎管狭窄症患者的疗效及对患者生活质量的影响。方法将62例颈椎管狭窄症患者随机分为观察组(31例)与对照组(31例),对照组采用传统单开门手术治疗,观察组采用单开门微型钢板内固定治疗。比较两组术中出血量、手术时间、JOA评分优良率,观察术后12个月颈椎曲度角变化和生活质量改善情况。结果术中出血量观察组少于对照组(P<0.05),两组手术时间比较差异无统计学意义(P>0.05)。患者均获得12个月的随访。术后12个月,JOA评分优良率观察组(90.32%)高于对照组(64.52%)(P<0.05);颈椎曲度角观察组高于对照组(P<0.05)。各项生活质量评分两组均较术前增加(P<0.05),且观察组高于对照组(P<0.05)。结论单开门微型钢板内固定治疗颈椎管狭窄症术中出血量少,疗效显著,且可改善患者生活质量。
Objective To investigate the efficacy of different posterior cervical spine surgeries for the treatment of cervical spinal stenosis and influence on the quality of life. Methods The 62 patients with cervical spinal canal stenosis cases were randomly selected. They were divided into observation group（ 31 cases） and control group（ 31 cases）. The control group was treated with traditional open surgery,and the observation group was treated with the single open door mini plate internal fixation. The volume of surgery bleeding,operation time,the excellent-good rate of JOA scores were compared. At postoperative 12 months,cervical curvature angle changes and the quality of life were observed. Results The volume of surgery bleeding in the observation group was less than in the control group（ P 0. 05）. There were no statistical difference in operation time between the two groups（ P 0. 05）. All patients were followed up for 12 months. After 12 month of follow-up,the excellent-good rate of JOA in the observation group（ 90. 32%） was higher than the control group（ 64. 52%）（ P 0. 05）. The anlge of cervical curvature in the observation group was higher significantly than the control group（ P 0. 05）. The life scores in both two groups increased significantly（ P 0. 05）,of which the observation group was higher than the control group（ P 0. 05）. Conclusions The single open door mini plate internal fixation for the treatment of cervical spinal stenosis can reduce the volume of surgery bleeding with obvious curative effect,and improve the life quality of patients.
