摘要
目的分析活血宁痛汤对血瘀型偏头痛患者临床疗效、证候积分及血液流变学的影响。方法回顾性分析2017年8月—2019年8月诊治的98例血瘀型偏头痛患者的临床资料。按治疗方法的差异分为研究组(n=50)与对照组(n=48)。对照组给予盐酸氟桂利嗪胶囊治疗,研究组给予活血宁痛汤治疗。比较2组临床疗效,以及治疗前后血液流变学指标、证候积分、头痛发作次数、持续时间、伴随症状、溶血磷脂酸(LPA)、肿瘤坏死因子-α(TNF-α)和不良反应发生情况。结果研究组临床总有效率高于对照组(P<0.01)。治疗后,研究组偏头痛证候积分、发作次数、持续时间及伴随症状改善情况优于对照组,全血高切黏度、全血低切黏度、血浆黏度、红细胞压积、红细胞聚集指数及红细胞变形指数改善程度均优于对照组,LPA及TNF-α水平低于对照组(P<0.05)。研究组不良反应发生率低于对照组(P<0.05)。结论活血宁痛汤治疗血瘀型偏头痛的临床疗效显著,可减轻疼痛症状,改善血液流变学指标,并调节血清LPA、TNF-α水平,安全性高。
Objective To analyze the effects of Huoxue Ningtong Decoction on clinical efficacy,syndrome score and hemorheology in patients with blood stasis migraine.Methods The clinical data of 98 patients with blood stasis migraine treated from August 2017 to August 2019 were retrospectively analyzed.According to different treatment methods,they were divided into research group(n=50)and control group(n=48).The control group was treated with Flunarizine hydrochloride capsule,and the research group was treated with Huoxue Ningtong Decoction.The clinical effects of the two groups were compared,as well as hemorheological indexes,syndrome scores,number of headache attacks,duration,concomitant symptoms,lysophosphatidic acid(LPA),tumor necrosis factor-α(TNF-α)and occurrence of adverse reactions before and after treatment.Results The total effective rate in the research group was higher than that in the control group(P<0.01).After treatment,the improvement of migraine syndrome score,number of attack,duration and concomitant symptoms in the research group was better than that in the control group,and the improvement of whole blood high-cut viscosity,whole blood low-cut viscosity,plasma viscosity,hematocrit,erythrocyte aggregation index and erythrocyte deformability index were better than those in the control group,and the levels of LPA and TNF-α were lower than those in the control group(P<0.05).The incidence of adverse reactions in the research group was lower than that in the control group(P<0.05).Conclusion Huoxue Ningtong Decoction has a significant clinical effect on patients with blood stasis migraine,which can alleviate pain symptoms,improve hemorheological indicators,and regulate serum LPA and TNF-αlevels with high safety.
