孕中期维生素D及微量元素水平与GDM发生的相关性分析 被引量:6

Correlation between the Levels of Vitamin D and Trace Elements in the Second Trimester and the Occurrence of GDM

摘要 目的分析孕中期维生素D及微量元素水平与妊娠期糖尿病(gestational diabetes mellitus,GDM)发生的相关性。方法选取2018年3月—2020年3月于本院进行产前检查的90例孕中期GDM孕妇(GDM组)和60例健康孕妇(对照组)作为研究对象,收集两组年龄、身高、孕次、病史、文化程度及血清铁蛋白(SF)、空腹血糖(FPG)、餐后血糖、空腹胰岛素、25羟维生素D[25(OH)D]、铁(Fe)、镁(Mg)、钙(Ca)、锌(Zn)、铜(Cu)、总胆固醇(TC)、甘油三酯(TG)、高密度脂蛋白(HDL)、低密度脂蛋白(LDL)等临床资料,应用Logistic回归分析探寻GDM发生的影响因素,并分析25(OH)D及微量元素水平与GDM发生的相关性。结果Logistic回归分析显示:糖尿病家族史、体质量指数、SF、25(OH)D、FPG、餐后2 h血糖、空腹胰岛素、TG、LDL、Fe、Zn、Cu是GDM发生的危险因素(P<0.01)。Spearman相关分析显示:25(OH)D与SF、FPG、餐后2 h血糖、TG、LDL呈正相关性,与空腹胰岛素呈负相关性;Fe与SF、FPG、餐后2 h血糖、空腹胰岛素、TG呈正相关性,与LDL呈负相关性;Zn与SF、FPG、空腹胰岛素、LDL呈正相关性,与餐后2 h血糖、TG呈负相关性;Cu与餐后2 h血糖、TG呈正相关性,与SF、FPG、空腹胰岛素、LDL呈负相关性。多元线性回归分析显示:25(OH)D、Fe、Zn、Cu水平与餐后2 h血糖、空腹胰岛素相关(P<0.01)。结论GDM孕妇孕中期25(OH)D、Fe、Zn水平低于正常孕妇,Cu水平高于正常孕妇,与GDM高风险相关。 Objective To analyze the correlation of vitamin D and trace element levels with gestational diabetes mellitus(GDM)in the second trimetrium.Methods Ninety pregnant women with GDM in the second trimester(GDM group)and 60 healthy pregnant women(control group)who completed prenatal examinations in our hospital between March 2018 and March 2020 were selected as the research subjects.Clinial data of two groups were collected,including age,height,gravida,history,education and the serum ferritin(SF),fasting plasma glucose(FPG),postprandial blood glucose,fasting insulin,25 hydroxy vitamin D[25(OH)D],iron(Fe),magnesium(Mg),calcium(Ca),zinc(Zn),copper(Cu),total cholesterol(TC),triglyceride(TG),high-density lipoprotein(HDL),and low density lipoprotein(LDL).Logistic regression analysis was used to explore the factors affecting the occurrence of GDM,and the correlation between 25(OH)D and trace element levels and the occurrence of GDM was analyzed.Results Logistic regression analysis showed that family history of diabetes,BMI,SF,25(OH)D,FPG,2 h postprandial blood glucose(2 h PBG),fasting insulin,TG,LDL,Fe,Zn and Cu were independent risk factors for the occurrence of GDM(P<0.01).Spearman correlation analysis showed that 25(OH)D was positively correlated with SF,FPG,2 h PBG,TG and LDL,but was negatively correlated with fasting insulin.Fe was positively correlated with SF,FPG,2 h PBG,fasting insulin and TG,but was negatively correlated with LDL.Zn was positively correlated with SF,FPG,fasting insulin and LDL,but was negatively correlated with 2 h PBG and TG.Cu was positively correlated with 2 h PBG and TG,but was negatively correlated with SF,FPG,fasting insulin and LDL.Multiple linear stepwise regression analysis showed that the levels of 25(OH)D,Fe,Zn and Cu were related to 2 h PBG and fasting insulin(P<0.01).Conclusion The 25(OH)D,HDL,Fe and Zn levels in the second trimester of GDM were lower than those in normal pregnant women,and Cu levels were higher than those in normal pregnant women,which were associated with a high risk of GDM.
