摘要
目的根据接种乙肝疫苗婴幼儿2周内HBsAg检测结果,筛选200例HBsAg低滴度阳性婴幼儿,分析其乙肝病毒DNA定量检测结果,探讨HBsAg阳性和疫苗接种的相关性及其临床意义。方法200例接种乙肝疫苗2周内化学发光微粒子法检出低滴度HBsAg阳性的婴幼儿为研究组,选取200例接种乙肝疫苗后HBsAg阴性婴幼儿为对照组,分别采用ELISA法和化学发光微粒子法进行HBsAg检测,每种方法选用同一台仪器;对2组婴幼儿分别进行乙肝病毒DNA定量(HBV-DNA定量)检测。结果研究组ELISA法对低滴度阳性HBsAg灵敏度很低,阳性率仅为6%,而化学发光微粒子法检出率为100%(P<0.05);研究组和对照组分别进行HBV-DNA定量检测,阳性率分别为2%和0%,差异有统计学意义(P<0.05)。结论接种乙肝疫苗的婴幼儿出现HBsAg高阳性率可能与疫苗接种相关,并非乙肝病毒感染所致,但部分阳性患者确系感染乙肝病毒所致,HBsAg结果弱阳性婴幼儿需HBV-DNA定量检测以确诊。
Objective According to the HBsAg test results of infants receiving Hepatitis B vaccine within 2 weeks,200 cases of HBsAg positive infants with low titers were screened,and the quantitative test results of Hepatitis B virus DNA were analyzed,and the correlation between positive and vaccination and its clinical significance were explored.Methods A total of 200 infants with low titers of HBsAg positive by chemiluminescence microparticle method within 2 weeks of inoculation with Hepatitis B vaccine were used as the research group and 200 HBsAg-negative infants were the control group.ELISA method and chemiluminescence microparticle method were used to detect HBsAg.The same instrument was selected for each method,and Hepatitis B virus DNA(HBV-DNA)was detected in two groups(HBV-DNA quantifiation).Results The sensitivity to low-titer positive HBsAg of ELISA method in the research group was very low with a positive rate of 6%,while the detection rate of the chemiluminescence microparticle method was 100%(P<0.05);HBV-DNA quantitative detection was carried out in the research group and control group respectively.The positive rates were 2%and 0%,respectively,and the difference was statistically significant(P<0.05).Conclusion The high HBsAg positivity in infants vaccinated with Hepatitis B vaccine may be related to vaccination and is not caused by Hepatitis B virus infection,but some positive patients are indeed caused by Hepatitis B virus infection.Infants with high HBsAg positive HBsAg need HBV-DNA quantitative test to confirm the diagnosis.
