电针结合康复训练对脑缺血再灌注大鼠大脑部位Nestin、bFGF、EGF表达影响 被引量:3

Effects of Electroacupuncture Combined with Rehabilitation Training on the Expression of Nestin,bFGF,EGF in Cerebral Areas of Cerebral Ischemia Reperfusion Rats

摘要 目的探讨电针联合康复训练对大脑中动脉缺血大鼠脑损伤的修复作用及其机制。方法健康SD雄性大鼠随机分为假手术组、模型组、电针组、康复组和电康组(电针+康复训练)共5组。每组24只,采用线栓大脑中动脉法制作大鼠局灶性脑缺血再灌注模型。电康组、电针组造模后4 h开始给予电针百会、大椎穴干预,每日1次,共10 d。电康组、康复组分别于造模后给予康复治疗,每天1次,连续10 d。干预结束后,检测各组缺血2 h再灌注3 d、7 d、10 d的治疗效果;免疫组化染色链霉亲和素-生物素复合物(strept avidin-biotin complex,SABC)法检测巢蛋白(Nestin)、碱性成纤维细胞生长因子(basic fibroblast growth factor,bFGF)和表皮生长因子(Epidermal growth factor,EGF)蛋白的表达。结果 Nestin免疫阳性表达明显增强;模型组大鼠术后3 d、7 d、10 d的Nestin、bFGF、EGF相对表达量均显著升高(P<0.01,P<0.05)。与模型组比较,电针组和康复组在3 d、7 d、10 d时促进Nestin表达的作用差异无统计学意义(P>0.05),电康组7 d时促进Nestin表达的作用较电针组和康复组差异有统计学意义(P<0.01)。脑缺血再灌注损伤后,在脑缺血侧bFGF和EGF程度表达较强,bFGF表达峰值出现相对较早,EGF表达峰值出现则较晚。与模型组比较,电针组、康复组、电康组3组在脑缺血皮质区内3 d、7 d、10 d不同时间点bFGF、EGF的表达数量增多,反应增强;电针组、康复组、电康组3组相比,电康组促进bFGF和EGF表达上调的效应更为明显(P<0.01,P<0.05)。结论电针结合康复训练在治疗脑缺血性疾病和改善缺血所致的神经功能障碍方面优于单纯的电针、康复疗法。这可能与电康法增加了大鼠缺血侧大脑神经干细胞的增殖及神经营养因子的表达有关。 Objective To explore the effect of electroacupuncture combined with rehabilitation training on brain injury and its mechanism in rats with middle cerebral artery ischemia. Methods SD healthy male rats were randomly divided into five groups:sham operation group,model group,electroacupuncture group,rehabilitation group and electroacupuncture+rehabilitation training group. The focal cerebral ischemia reperfusion model was established in 24 rats in each group by using the middle cerebral artery method. The Diankang group and the electroacupuncture group were given Baihui and Dazhui acupoint intervention at 4 h after modeling,once a day for a total of 10 d. The Diankang group and the rehabilitation group were respectively given rehabilitation treatment after modeling,once a day for 10 consecutive days. At the end of intervention,the therapeutic effects of ischemia for 2 h and reperfusion for 3 d,7 d and 10 d were tested. Immunohistochemical staining SABC method was used to determine the expression of Nestin,basic fibroblast growth factor(bFGF)and epidermal growth factor(EGF)proteins. Results The positive expression of Nestin was significantly enhanced. The relative expressions of Nestin,bFGF and EGF in the model group were significantly increased on 3 d,7 d and 10 d after operation(P<0.01 and P<0.05). Compared with the model group,there was no significant difference in the effect of promoting Nestin expression between the electroacupuncture group and the rehabilitation group at 3 d,7 d and 10 d(P>0.05),while the effect of promoting Nestin expression in the Diankang group at 7 d was significantly different from that in the electroacupuncture group and the rehabilitation group(P<0.01). After cerebral ischemia reperfusion injury,bFGF and EGF were strongly expressed on the ischemic side of the brain. The peak expression of b FGF was relatively early,while the peak expression of EGF was relatively late. Compared with the model group,the expression of bFGF and EGF in the three groups of electroacupuncture group,rehabilitation group and Diankang group increased at different time points of 3 d,7 d and 10 d in the cerebral ischemic cortex. Compared with the electroacupuncture group,the rehabilitation group and the Diankang group,the effect of promoting the up-regulation of bFGF and EGF in the Diankang group was more obvious(P<0.01,P<0.05).Conclusion Electroacupuncture combined with rehabilitation training is superior to pure electroacupuncture and rehabilitation therapy in treating cerebral ischemic disease and improving the neurological dysfunction caused by ischemia. This may be related to the increased proliferation of neural stem cells and the expression of neurotrophic factors in the ischemic side of the brain.
