分析对比脑出血患者的CT及MRI影像学特点 被引量:5

摘要 目的:对比分析CT及核磁共振(MRI)在脑出血诊断中的影像学特点。方法:选取我院2017年3月—2019年5月收治的脑出血患者116例,所有患者均进行CT及MRI检查。观察脑出血诊断中CT及MRI检查的影像学特点、水肿现象检出率、脑出血患者的病灶形态及分布特征,以术后病理学检查为金标准。结果:本次研究共116例患者,CT检出基底节出血71例、脑叶出血13例,脑干出血12例,小脑出血9例,脑室出血4例;MRI检出基底节出血72例、脑叶出血14例,脑干出血14例,小脑出血9例,脑室出血5例,MRI检出率与CT检出率差异无统计学意义(P>0.05)。脑出血CT影像学表现下,可见患者脑内清晰边界,皆为均匀的高密度灶,病灶主要形态为肾形。在增强CT下可见病灶环形特征强化,靶向特征显露。脑出血MRI检查脑内边界较为模糊,影像学表现以混合信号特征为主,T1为等信号,T2均为高信号表现,T1环形为稍低信号,T2环形为高信号。结论:在脑出血诊断中,CT及MRI检查均可见明显影像学特点,不同位置不同时间CT及MRI影像学表现皆各有特征,MRI扫描敏感度高于CT检查,二者均有良好的诊断作用,需根据患者情况进行检查方式的选择。 Objective To compare the imaging features of CT and MRI in the diagnosis of cerebral hemorrhage.Methods 116 patients with cerebral hemorrhage from March 2017 to May 2019 in our hospital were selected.All patients were examined by CT and MRI.To observe the imaging characteristics of CT and MRI in the diagnosis of cerebral hemorrhage,the detection rate of edema,the focus morphology and distribution characteristics of patients with cerebral hemorrhage,and take the postoperative pathological examination as the gold standard.Results There were 116 cases in this study,71 cases of basal ganglia hemorrhage,13 cases of lobar hemorrhage,12 cases of brainstem hemorrhage,9 cases of cerebellar hemorrhage and 4 cases of ventricular hemorrhage were detected by CT;72 cases of basal ganglia hemorrhage,14 cases of lobar hemorrhage,14 cases of brainstem hemorrhage,9 cases of cerebellar hemorrhage and 5 cases of ventricular hemorrhage were detected by MRI,there was no significant difference between MRI and CT(P>0.05).Under the CT imaging of cerebral hemorrhage,the clear boundary of the brain was seen,all of them were homogeneous high-density foci,and the main shape of the foci was kidney shape.In contrast-enhanced CT,the ring feature of the focus was enhanced and the target feature was revealed.In the MRI examination of cerebral hemorrhage,the boundary of the brain is relatively fuzzy,the imaging features are mainly mixed signal,T1 is equal signal,T2 is high signal,T1 ring is slightly low signal,T2 ring is high signal.Conclusion In the diagnosis of cerebral hemorrhage,CT and MRI can show obvious imaging characteristics.The imaging manifestations of CT and MRI in different locations and at different times have their own characteristics.The sensitivity of MRI scanning is higher than that of CT scanning.Both of them have a good diagnostic effect.It is necessary to choose the examination method according to the patients’conditions.
