摘要
目的比较子宫输卵管造影(HSG)与经阴道超声对输卵管积水的诊断价值。方法选取2018年8月~2019年7月于潍坊医学院生殖医院就诊并确诊为输卵管积水的189例患者作为研究对象,所有患者均接受HSG、经阴道超声和腹腔镜输卵管通液术检查。以腹腔镜输卵管通液术作为金标准,比较HSG和经阴道超声诊断输卵管积水的准确率、特异度、灵敏度以及诊断不同程度输卵管积水的阳性率。结果HSG诊断输卵管积水的准确率、灵敏度(88.52%,86.51%)较经阴道超声(67.79%,61.94%)高,差异有显著性(P<0.01),HSG诊断输卵管积水的特异度(97.06%)与经阴道超声(92.65%)相比,差异无显著性(P>0.05)。HSG对于轻、中度输卵管积水诊断的阳性率分别为89.55%,83.51%,明显高于经阴道超声的13.43%,55.67%,差异有显著性(P<0.01),对于重度输卵管积水,两者诊断的阳性率分别为87.20%,92.80%,差异无显著性(P>0.05)。结论HSG对输卵管积水,尤其是对轻、中度输卵管积水的诊断价值高于经阴道超声,建议将HSG作为诊断输卵管积水的首选方法。
Objective To compare and analyze the diagnostic value of hysterosalpingography(HSG)and transvaginal ultrasound in hydrosalpinx.Methods A totle of 189 patients diagnosed with hydrosalpinx who were admitted to Reproductive Hospital of Weifang Medical University from August 2018 to July 2019 were enrolled.All patients were examined by HSG,transvaginal ultrasound and laparoscopic surgery.Laparoscopic surgery is used as the gold standard to compare the accuracy,specificity and sensitivity of HSG and transvaginal ultrasound of hydrosalpinx,and record the positive rate of different degrees of hydrosalpinx.Results The accuracy and sensitivity of HSG in diagnosis of hydrosalpinx(88.52%,86.51%)were higher than transvaginal ultrasound(67.79%,61.94%),which has statistically significant difference(P<0.01).Compared with transvaginal ultrasound(92.65%),the specificity of HSG in diagnosis of hydrosalpinx(97.06%)was not statistically significant(P>0.05).The positive rates of HSG for mild and moderate hydrosalpinx were 89.55%and 83.51%,respectively,which were significantly higher than transvaginal ultrasound(13.43%and 55.67%).The difference was statistically significant(P<0.01).For severe hydrosalpinx,the positive rates were 87.20%and 92.80%,respectively,and no significant difference was found(P>0.05).Conclusion The diagnostic value of HSG for hydrosalpinx is higher than transvaginal ultrasound,especially for mild and moderate hydrosalpinx,and it is suggested that HSG should be the prior choice for hydrosalpinx.
