摘要
目的:探讨穴位放血配合针刺对肝郁痰凝型乳腺增生症患者的临床疗效.方法:随机选择2017年3月-2018年12月本院体检转入中医科门诊诊治的120例乳腺增生症患者为本次研究对象,根据治疗方法不同分为对照组和干预组,每组60例.对照组给予枸橼酸他莫昔芬片,维生素E胶丸,复合维生素B片.干预组在对照组治疗基础上给予穴位放血配合针刺治疗.评价两组临床疗效、中医症状积分及不良反应发生情况.分别于治疗前、治疗后1个月,采集患者血液标本,检测血中雌二醇(Estradiol,E2)、孕酮(Progesterone,P)、催乳素(Prolactin,PRL)水平.比较两组治疗后的生活质量评分.结果:干预组治疗总有效率明显高于对照组,差异有统计学意义(P<0.01).干预组治疗后乳房疼痛积分、肿块大小积分、肿块质地积分、证候总积分均显著低于治疗前和对照组治疗后(P<0.05).干预组治疗后血清E2、PRL水平均明显低于治疗前及对照组治疗后,P水平明显高于治疗前及对照组治疗后,差异有统计学意义(P<0.05).干预组治疗后躯体功能、心理功能、社会功能、认知功能评分及生活质量测定量表(QLQ-C30)总分均显著高于对照组,差异均有统计学意义(P<0.05).两组不良反应发生率差异无统计学意义(P>0.05).结论:通过穴位放血联合针刺疗法能够明显改善乳腺增生症的临床疗效,调节激素失调的状态,提升患者生活质量.
Objective:To explore the clinical effect of acupoint bloodletting plus acupuncture on brest hyperplasia of the Ganyu Tanning type(肝郁痰凝型).Methods:120 patients were divided into the control group and intervention group according to different treatment methods,60 cases in each.The control group was given tamoxifen citrate tablets,vitamin E capsules,and multivitamin B tablets.The intervention group was treating by acupoint bloodletting plus acupuncture more.The clinical efficacy,TCM symptom scores and adverse reactions were evaluated in both groups.Blood samples were collected from patients before and 1 month after treatment,and the levels of E2,P and PRL were measured.The quality of life scores were compared between groups.Results:The total effective rate in the intervention group was significantly higher than the control group(P<0.01).The breast pain score,mass size score,mass texture score,and total syndrome score in the intervention group were significantly lower(P<0.05).The levels of serum E2 and PRL in the intervention group after treatment were significantly lower than before treatment,and those in the control group after treatment,and the levels of P were significantly higher than those before treatment,and those in the control group after treatment.The physical function,psychological function,social function,cognitive function score and QLQ-C30 total quality of life score in the intervention group were significantly higher than the control group after treatment,with statistically significant differences(P<0.05).There was no statistical difference in the incidence of adverse reactions between groups(P>0.05).Conclusion:Acupoint bloodletting plus acupuncture can significantly improve the clinical efficacy of breast hyperplasia,regulate the state of hormone imbalance,and improve the quality of life of patients.
