摘要 目的:探究Er∶YAG激光联合盐酸米诺环素治疗药物性牙龈增生的疗效及对患者龈沟液中TGF-β1、IL-1β水平的影响。方法:纳入108例药物性牙龈增生患者,随机分为3组(n=36)。3组均予以牙周基础治疗,观察组Er∶YAG激光联合盐酸米诺环素治疗,对照A组Er∶YAG激光,对照B组盐酸米诺环素治疗,每周1次,连续5周。5周后统计3组治疗总有效率、不良反应发生率,并对比治疗前、疗程结束后牙龈增生指数(GHI)、出血指数(BI)、牙周探诊深度(PD)改善情况及龈沟液TGF-β1、IL-1β水平。结果:观察组GHI、BI、PD低于对照A、B组(P<0.05,A、B组相比,P>0.05);观察组治疗总有效率高于对照A、B组(P<0.05,A、B组相比,P>0.05);观察组TGF-β1、IL-1β水平较对照A、B组低(P<0.05,A、B组相比,P>0.05);3组不良反应发生率无明显差异(P>0.05)。结论:Er∶YAG激光联合盐酸米诺环素治疗药物性牙龈增生,可促进患者症状改善,降低TGF-β1、IL-1β水平,提高疗效,安全性高。 Objective: To investigate the therapeutic effects of Er∶YAG laser combined with minocycline hydrochloride on drug-induced gingival hyperplasia and on the level of TGF-β1 and IL-1β in gingival crevicular fluid(GCF). Methods: 108 patients with drug-induced gingival hyperplasia were divided into 3 groups(n=36) and treated by Er∶YAG laser combined with minocycline hydrochloride(the test group), Er∶YAG laser(control A) and minocycline hydrochloride(control B) respectively following basic periodontal treatment. The adjuctive treatments were conducted once a week for 5 weeks. Before and after treatment periodontal indexes of GHI, BI, PD and TGF-β1 and IL-1β level in GCF were measured. Data were statistically analyzed. Results: After treatment, the GHI, BI, and PD in the test group were lower than those in group A and B(P<0.05, between groups A and B,P>0.05);The total effective rate of the test group was higher than that of group A and B(P<0.05, between group A and B,P>0.05). After treatment, the levels of TGF-β1 and IL-1β in the test group were lower than those in group A and B(P<0.05, between group A and B, P>0.05). There was no significant difference in the incidence of adverse reactions among the 3 groups(P>0.05). Conclusion: Er∶YAG laser combined with minocycline hydrochloride in the treatment of drug-induced gingival hyperplasia can increase the treatment effects, reduce the levels of TGF-β1 and IL-1β in GCF.
